Can anyone help me with Zantac dosage for a 5 week old that weighs 11-12 pounds? How do I figure the mg/kg thing? Thanks!

I'll try to help you, Laura is our guru with this but she works during the day, she will I hope correct me if I am wrong!
The max dose you can give an infant is 10mg/kg... so.... 11-12 lbs (I'll use 11.5) is 5.2kg (1kg = 2.2 lbs) you take 5.2 x 10= 52mg/day Your bottle will tell you have many mg/ml but it is usually 15mg/ml So 52mg/day would equal 3.46ml/day or round up to 3.5ml/day split however you wish either in 2 or 3 doses. So how should I decide when to give him Zantac & how much & when to give the Previcid? I didn't know I could give different dosing amounts! That would be great for nighttime!

Hi! I'm new to the boards and just reading through some posts. It seems as though according to the dosages I've been seeing on here that my LO's is too low. He's been on zantac for a couple weeks with no noticeable difference. He is 2.5 months and 13.5 lbs. The doctor has prescribed him .7ML of zantac twice a day. It is 15 MG/ML. Hmmm...guess I'll be making a call! I had no clue and just figure the doctor knows best. We were just at his 2 month appt. on Friday and talked about his reflux. Interesting she didn't talk about the dosage! She just said to finish this bottle and if we don't see a difference then we'll move onto another medicine.
Am I right that he should be getting a higher dose?

the thing about zantac is that it's very weight sensitive and most babies seem to 'plateau' on it...your dr might be going through the motions w/ zantac, bcse some insurance companies insist you try zantac first before moving on to another type of med (PPI=proton pump inhibitor like prilosec, prevacid, nexium, protonix)
as you wait for a PPI, it most certainly makes sense, in my opinion, to give zantac at the highest dose for weight...even if/when you do make the switch to a PPI, it can take up to two weeks for the PPI to have 'show' it's full effects so while you wait it's important to continue along w/ the zantac (making sure to space it 4hrs from the PPI so the two drugs don't cancel each other out) here's all about zantac dosing: Zantac dosing- my/other’s question (has axid & pepcid dose,
too): http://www.infantreflux.org/forum/forum_posts.asp?TID=8288&PN=1 conversion chart: http://www.sfca.co.uk/match/kilolbs.htm 13.5lbs = 6kilos (from above link) so 6 x 10 = 60mgs, total for the day of zantac your bottle should read 15mgs/ml so 60 divided by 15 = 4mls total for the day of zantac you can divide this into 2 'uneven' doses if you want to for ex: if night time is worse, give a higher portion before bed also: TELL your dr, don't ask . read the info from the link above (http://www.infantreflux.org/forum/forum_posts.asp?TID=8288&PN=1) and follow what it says (ie: talking to your pharmacist) and then once you have the facts, just 'tell' your ped you're increasing the zantac dose and that you would like to try a PPI, too (www.marci-kids.com will tell you the PPI dose to request) Edited by hellbennt - 25 October 2009 at 10:33pm |

Thank you for the great info! I think I'm calling the doc today because his reflux seems to be bad the past couple days. Poor thing sounds like a little piggy trying to breathe. She mentioned that next we would try an antacid. I haven't done too much research yet but I think I'd almost want to go straight to a PPI instead? Trying to get him to sleep is totally exhausting...then he's up screaming after 30 minutes on the dot!
