My daughter is 6 months old and was diagnosed with reflux when she was 3 weeks old.  At the end of October an Upper GI was done because Claire was choking in her sleep, during play time and while she was eating or basically all the time.  It was determined at that point that she has Stage 3 reflux or as her Ped says, the worse he has ever seen in an infant.  Thankfully she is a happy baby so some of the meds she is on are working.

My concern with her current medicine is that I am not sure if it is the right dose or combination of meds.  Right now she is on 3 ml of Prilosec twice a day and 1.5 ml of Zantac twice a day.  She has been waking up in the middle of the night coughing for the last two weeks.  It seems like the coughing is because of post nasal drip from the reflux.  Her nose is constantly stuffy and I have been trying to clear it with Simply Saline.  We have also added rice cereal to her formula and once we receive the Thick It we will be adding that to breastmilk.  Claire was on Prevacid from 3 weeks old up until she was 6 months old a few weeks ago.

We are going to see a Ped GI in a couple weeks finally because I don't think her regular Ped is very skilled in this area.  They told me it was fine to give her the Prilosec and the Zantac at the same time!  I am so thankful I found this board and learned better.  Any input on dosage amounts or timing would be so helpful!  I also feel like the Prilosec isn't really working and that the Prevacid is much better.  Has anyone else ever experienced this?

hi & welcome!!
my first thought is that the liquid prilosec (called a compound) isn't being made properly, so that if it works at all, it works best for 2 weeks of the month and then things might start to go down hill from there?
also the next thing to do would check the dose at www.marci-kids.com
to see if it's a proper dose of PPI.
check here, Prevacid 101, last post on the page (it has info about prilosec, too and the link to check the zantac dose is there, too)
http://www.infantreflux.org/forum/forum_posts.asp?TID=1936&PN=0&TPN=1
you can also check zantac dose there, too.

you should space the zantac & PPI 4hrs apart, but some (including myself) have had success w/ only 2hrs. you'll have to 'see' what works for your daughter...

I would also (demand!) a Swallow Study to check the mechanisms of swallowing and check about the choking! this is not the same as an Upper GI...this will look at HOW she is swallowing and see if there is any aspiration going on. you will also learn the exact thickness to thicken w/ ST, if it's necessary...
