Inactiveok, help me out here… why are others able to get 30 mg of Prevacid but I can’t?
My dd will be 11 weeks old on Friday. She weighs 9 lbs 13 oz as of an hour ago. We were just at the doctor because she’s roller coastering… We’ve tried 15 mg once a day and we’ve tried 7.5 mg twice a day. With both of them it does not seem to carry her the full time. Like the 7.5 mg takes her about 8 hours, not 12 and the 15 mg isn’t even taking her 14 hours… I’ve dismissed her discomfort as other things long enough but last night she was stiff and arching her back. This was about 14 hours past her 15 mg dose. It was 4.5 hours since her last feeding and she acted hungry but then would only take a little at a time and then push it away.
I’m having a hard time understanding the marci-kids dosing info because it isn’t talking about the 15 or 30 mg. and it’s by age not weight. But what the doctor was telling me does sound more similar to what that site is talking about…. And they even had a nurse practitioner from Children’s Hospital GI department there today and she also said that Anika is more than maxed out on the Prevacid dose.
So again… HOW are the rest of you getting 30 mg??? I really feel as though we need more. We had her on the 7.5 mg twice a day until Saturday and then we switched to 15 mg once a day and she did really good Saturday. It’s like I know she needs the higher dose because that was like 7.5 mg on Friday PM and then 15 mg on Saturday AM…
Oh, HELP!

InactiveAidan’s doc just upped his dose at 3 months because the 15 wasn’t working well enough…he was very big though, 16 lbs.I guess I was lucky, I never asked her to do it, she just did. I think it IS a weight thing, there’s no way she would have put him on 30 if he hadn’t weighed so much.

InactiveThanks Jill. That partially explains it but they were indicating that we would need to be DOUBLE her weight before they would do it. Meanwhile, I thought I saw kids on here right around the same age and weight who were already on the 30 mg…
Like I said, I’m not understanding the dose info on Marci-kids very well either.
I jusy don’t know what to do.
I only see my baby a few waking hours a day on work days and it SUCKS for those hours to be with her in pain that is clearly from reflux. We went through this with my ds (I just posted my introduction yesterday if you care to read the novel) and I just don’t want the same memories or dread of feeding my kid…We’re already putting cereal in all of her bottles now. The ratio is 1 tsp per 1.5 oz of formula. Should we do more? That appears to be my only option.
I just wanted to cry in the doctor’s office today because it was a waste of a visit… And I know going to the GI would be the same. The GI never really did anything for us with ds anyway…October 11, 2006 at 11:09 am #15386Anonymous
InactiveI’m so sorry that your doctor isnt being helpful….it does suck to watch kids in pain and feel helpless….are you happy with the doc overall? have you showed the doc the dosing stuff from marci-kids? maybe he/she could explain it, and maybe then they would realize that the meds need to be upped….
good luck!

Inactivethe marci-kids.com info IS by weight, maybe you misunderstood it.
it also has a dosing calculator that includes both the 15 and 30 mg. take a closer look.
maybe you can show your doctor that info.
also, she is not maxed out on the dose at 15 mg. they all say that 🙄here is the chart:
Age Dose per day (mg/kg/dose) < 3 months 1.5 to 1.75 mg/kg three times per day 3-6 months 1.25 to 1.5 mg/kg three times per day 7 months to 2 years 1 to 1.25 mg/kg three times per day > 2 years 1 mg/kg two or three times per day > 5 years 0.75 to 1 mg/kg two times per day so say your child is bewteen 3 and 6 months. you do 1.5 mg times their weight in kg and that gives you how many mg and you times that 3 times a day.
the calculator is much easier you just type in age and weight.nataliachick72006-10-11 11:15:15

InactiveWell that’s the thing Shannon… I don’t really understand what the marci-kids site is saying. It doesn’t talk about 15 mg or 30 mg… But what the doctor was saying sounds more similar to what the site says… And she had the person from the children’s GI office there so I know going to the GI isn’t going to get us anything more…
Overall I am happy with the pediatrician’s office (all except for one that I refuse to ever see again – -see my intro and you will see why). They are close and they have good hours and many doctors so it is easy to get in… and I suppose they are responding to us some on this reflux but I’m still unhappy that we can’t up her dose. I don’t understand why.

Thanks for responding though Shannon…

InactiveThanks Natalia
I tried the calculator but it still didn’t make sense….

OK, this is what it says for Anika at 3 months, 10 lbs,
Your child, who is 3 months old and weighs 4.5 kg, should receive a dose of 5 to 7 mg of Prevacid delivered 3 times per day for a total of 15 to 21 mg per day.
So it’s like we need 7.5 mg 3 times a day, not twice a day! I don’t see HOW I can get them to do this though… Nor do I know how WE will do it because daycare won’t give meds…

InactiveTHanks!

I’m on my way out right now (Christmas shopping – – LONG story that involves a family wedding in 10 days and the rest of them stressing me out to “save on shipping…”) I’ll be back in a bit.

InactiveLeanne,
It wasn’t easy for me to get Myles on the higher dose he needed. In fact, it took me nearly two months to talk his doctor into trying the marci-kids dose. He was miserable for the entire time his dose was too low and it was killing me because I knew that if he had the right dose he’d feel better.
I was so frustrated and ready to take him out of state to a gi (we only have one in my state and he’s lousy). I tried one last time. I printed and faxed the marci-kids information to my son’s doctor and then called her daily to report how bad things were with Myles and she finally caved in and let me try the high dose. Within days he was better and by the tenth day he was a new baby.

Is your son’s doctor a pediatrician? Some moms have reported that they had better luck getting a high ppi dose from a family physician. Maybe they are just more comfortable and knowledgeable about PPI’s and reflux than peds are. Myles’ doctor is a family physician.
Your right that she probably nees 7.5 mg / three times daily. Keep trying. Many of us have had to fight hard to get the high doses our babies needed.
Good luck and please keep us posted.

InactiveMany doctors say that they are maxed out on meds because they’re using the information supplied to them from the drug companies which do conservative dosing. The marci-kids info is different because it’s based on reviews of research that they’ve done indicating the need for more aggressive dosing in many infants and children. Our docs also wouldn’t give us a higher dose, and I had to go from doctor to doctor until someone would give us 30mg. If you are having a hard time and would like your doctor to try the marci-kids dosing, you might be able to get Dr. Phillips to talk to your doc for you about the safety of this. Alternately, there’s a sheet to take to your doctor explaining where the dosing guidelines came from on their website… it’s on the left top hand side of the page entitled “PPI dosing memo”. Good luck. I hope that something works out for you soon.

Inactiveleanne—-it sounds like your doc is already using the marci-kid info. you are rounding everything up and wanting to base your meds on that. your baby will only be 11 weeks on friday—-which is not 3 months. and she does not weigh 10 lbs yet—-and in infants, ounces really do make a difference. how long has your daughter been on the 15 mgs of prevacid? it sounds like it is working for her, but just not lasting long enough. what about asking for a script of carafate to help with the fussy times?kevieb2006-10-11 15:2:2

InactiveSorry about that. I was reading so quickly that I didn’t realize her age and weight. Along the same lines as Christine’s thought, could you use some mylanta?

InactiveThank you everyone!!! It’s nice to have people behind me…
Last night dh and I printed out as much as we could off of the marci-kids site and we read it and calculated what we think should be Anika’s dose. I’m all set to fax over a letter to the doctor as well as the Home page, the general information page, the page that lists the Marci-kids team (physicians, research and support staff), biographies on Dr. Bothwell & Dr. Phillips, the 5 page “paper” on the PPI dosing, and finally the PPI dosing chart. I’m going to ask them to read it all and then consider agreeing to us trying 7.5 ml 3 times a day and if it works then we would ask them to write a Rx for “two 15 mg a day.” We will only use 7.5 mg three times a day but our insurance is such that we have to get “prior approval” for meds so it’s already a big hassle to change/ get new Rx’s. But as I calculate the dosing, as she gets older and gains weight, she may end up needing up to 30 mg’s a day but I don’t believe she does now…
We’ve been on 15 mg’s for about a month now. We’ve tried doing it once a day, twice a day, strict timing, etc. In a nutshell though, it seems as though a 15 mg dose lasts her about 14 hours (not 24) and two 7.5 mg doses lasts her about 8 hours (not 12). But what is happening is that by the time we give her a dose, she is already in pain… so even waiting an hour to feed she is still screaming and refusing. Like last night she took 3 oz. at 10 PM. She woke at 4 AM and was sort of fussing but I didn’t want to give her the meds yet because I know they are not lasting long enough through the evening… But I know that’s what she wants because when I would give it to her at 4 AM she happily falls asleep but she didn’t this morning, she continued to stirr. I gave her 15 mg’s at 5:30 AM and tried to feed her at 6:30 AM. She screamed. By 7:15 all I had gotten in her was half an ounce…
This just isn’t right. She is in pain. She is rollercoastering. She may be getting releif some of the time but not all the time and that has to be causing damage. I feel as though my son had long lasting damage because of not being diagnosed… I refuse to let my daughter go thorugh pain like this when there is research out there showing that she could get a higher dose. I told dh this morning that if our doctor won’t do it then I will find one that will… I will fly to Missouri… I will do whatever I have to…

– – Oh, as for the Mylanta. We do give that to her some but 1- we really have NO CLUE on the dosing amount and 2- the pediatrician even said that it isn’t a long term fix but just an immediate band-aid.

InactiveMylanta never worked for Hailey. You’re right- they shouldn’t be in so much pain that they refuse to eat. If you’re seeing feeding problems then it’s best to get things under control as soon as possilble. Hopefully all the info you’ve sent to the doc will convince them. Good luck. I hope that they listen.
