InactiveAs many of you know I got Anika’s doctor to let us try three 7.5 mg doses of Prevacid a day. We started that on Thursday (well, 15 mg in the AM and 7.5 mg in the PM) and she did seem to be doing better. But then Saturday was odd…
I suppose several things tripped up that day: 1- somehow we ended up with a level 1 nipple so she gave up after 2 oz. 2- we ended up being about 3 hours late on her 2nd dose of meds 3- my son dropped a plastic container filled with legos on her (possibly her head?
) 4- I slipped in the kitchen and although my knee took the bulk of the fall she bumped the floor. I was SO WORRIED about her and the bumps and her lack of eating did not help! But we were around some friends (4 other Mom’s – – one a nurse) and they all assured me that she was alert, happy, giggly, making eye contact, etc. but then they all noticed how she seemed real stiff. I had hoped it was just a combination of everything including not having enough meds so when we got home I gave her 15 mg’s. 30 minutes later I gave her some Mylanta, and then I gave her a really warm bottle and she took 4 oz. so that was good.Sunday went a lot better and we got her doses better timed but then her feeding times were a bit off (more because we all slept in and got a bit off). But then yesterday at daycare they only got her to take 2.5 oz at 10:20 AM and 2 oz at 2:30 PM. They said she seemed real stiff and acted as if maybe the flow was too fast? Who knows, maybe dh didn’t shake the formula before he made her bottles? By the time I got her at 5:30 she was of course STARVING! Once I calmed her down she did take 3 oz. plus she ate a little ceral on a spoon and at 11 PM she took 4 oz really quickly and easily. It was one of those rare soothing bottles. It was also warm to hot so maybe that was part of the difference?
Anyway, what do you all think? THose of you that got on the marci-kids dose how long did it take for things to normalize for you?
Also, anyone know what is an appropriate dose of Mylanta? I really don’t know for Anika or for Maxon so we just guess…

InactiveBoy, I’ve had days like that….where everything and anything goes wrong.

With Myles I noticed a difference in a few days when his meds were upped, but the real change didn’t happen until the 10th day. Her dose (15mg) was awfully low, so it may even take closer to two weeks for the full effect.
I’m not sure, but I think Joel (who used to work at marci-kids) said not to give Mylanta within two hours of a Prevacid dose. Does anyone else remember that, or am I confused?

Are you giving the Prevacid at least two hours after a meal and following it up 30 minutes later with a full meal?
Also, I wonder if three 7.5 mg doses would work better than the two doses you giving her now. Myles really needed three doses.
As for how much Mylanta, I didn’t know about it until Myles was 4 months old. At that age I gave him 2 mls up to 4 times per day. Supposedly too much will cause loose stools, but otherwise it’s pretty safe.

InactiveYeah, we are giving her 3 doses of 7.5 ml each. It was just that first day when I gave her 15 ml in the AM, got permission that day from the doctor to give her more so then we gave her 7.5 ml that evening. After that it has been 7.5 ml around every 8 hours… well, except for Saturday when we were late with one. I even talked daycare into giving her one dose a day at least while we are on this trial period so that they are timed as close to every 8 hours as possible.
Yes, we are trying to follow the “meds no sooner than 2 hours after a meal and feedings 30 minutes after meds” rule. As you all know, that does get hard to time though… I’m finding that sometimes we are ending up with the feeding closer to an hour afterwards because of the timing of stuff… and I figured that was better than the other way (her screaming to eat immediately after I give her the Prevacid and me giving in to that).
So Christine do you really think that the 15 mg a day was that low? I guess compared to the kids on 30 mg a day it is. I feel lucky to have talked the doctor into letting us TRY the 3 doses of 7.5 mg but I have to go back to them next Tuesday to discuss her progress. If she needs more than what she is currently getting I fear as though they won’t believe me on this and just leave us at 15 a day.
Breathe.
I just need to be patient. You are probably right that she needs at least until next week for it to take full effect. I guess we just get excited about the potential for improvement that we want it right away. I’ve been doing some more reading/ posting and I saw what you wrote about cereal in the bottles:
Thickening is controversial. Some say it helps keep things down. Does he throw up/spit up alot? Some (like me) use it because their babies choke on thinner feeds. Some use it because of slow weight gain, and some say it helps keep acid down and reduce pain. It’s really trial and error. I personally wouldn’t thicken unless absolutely necessary because it often causes more problems, like gas, and consipation.
I wonder if now that Anika is on a higher dose of Prevacid if maybe she doesn’t need the cereal? What are your (or anyone else’s)thoughts? Anika’s reflux is definitely more of a “silent” form but she does have mini spit-up’s every now and then. Her weight really is fine.
Gah, when you talk about it being trial and error it sure is… I could obsess over this all day!

I guess I’d probably stop the cereal and see how she does. It really made no difference for Myles whatsoever, except that he choked during feedings and it helped a little with that. I eventually stopped thickening because I felt he needed more breast milk/formula since he was not a big eater.
If her weight is fine, she doesn’t choke, and she doesn’t spit up much…IMO she probably doesn’t need the cereal.
Yeah, I know what you mean about obsessing. I used to contemplate what to do about Myles’ reflux nearly every waking moment of my life and then I’d dream (or I should say have nightmares) about it at night, too.

Well, sort of… I guess it could be clearer!Christine, I was thinking about this thread yesterday and started paying more attention to the timing of the meds in relation to her meals instead of necessarily trying to be close to every 8 hours. I’m thinking that she almost does better to take the bottle an HOUR after the meds. She also does really good when the bottle is warm to hot. Daycare only got 2.5 oz. in her yeserday AM but I sent a e-mail around lunch time and in the afternoon she took 4 oz – – they said she drained the bottle so dry they contemplated opening the extra formula I had sent.

I think you might be right about the ceral but now I think we’ve got a good run going here (amazing the difference that 24 hours can make!) and I want to keep this going at least until our appointment with the doctor next week… You know, not changing too much at once. Then we will look at cutting out the cereal. We are actually travelling tomorrow (driving from Detroit, MI area to Nashville, TN) so she may end up getting a few bottles without the ceral anyway.

I think that’s probably a good idea…it’s never good to make too many changes at once, because then you never know what’s working and what’s not.
As for the timing, Joel did say not to wait more than an hour, so if an hour is working, that’s great, but I wouldn’t stretch it out any more than that. Also, when Myles was on three doses I never spaced them 8 hours apart because he was sleeping most of the night by then. I gave him all three doses during his daytime waking hours (between 6:30 am and 7:30pm) and that worked fine for him.
Good luck. I hope she’s feeling better soon and good luck with your travels. I often have made changes that I was afraid to make while traveling and found that they worked fine.

InactiveYeah see… I think we were so focused on giving the meds every 8 hours when really it’s in relation to the meals that makes them more effective. I’m just not so sure what we will do after this trial period. Currently we are giving her a dose in the AM, then daycare gives her a dose around 1 PM and then we give her one around 9 – 9:30 PM. But I told daycare that after the trial period we would then try to take care of all of the dosing at home. I’m not sure if this will work all that well though… I mean, we give her the first AM dose and it covers 2 feedings (the 6-ish AM and the 10-ish AM feeding). Then daycare gives her meds at 1 PM and it works for 2 more feedings (the 2-ish PM and the 6-ish PM). Finally we give her the 3rd dose and it covers that last feeding around 10 PM. I just don’t think the AM dose will carry her through 3 feedings, do you? Plus, by the time I pick her up from daycare she is usually hungry and we don’t really have 30 minutes to play with… oh, and if we are running late or she just acts hungry they might start that 6 PM bottle and then we are really messed up.
Hmmm, I might have to ask daycare if they could continue this….

InactiveYeah, I’m thinking of asking the owner what her thoughts about it are… Now that they have done it for a few days I think they will agree that it’s pretty easy to do.
Well, I’m off to head home and finish packing. We are crazy but we are driving to TN for a family wedding this weekend. I hope to be back on Monday and to report some good progress for us.
I’ll be praying for the same for all of you! -
