InactiveHi,
I’m just wondering if anyone else using PPI’s has noticed that they seem less effective over time?
David started on Zegerid at 4 months old. After just a few days I noticed a dramatic difference. He took a two hour nap laying down one day which he’d never done before (unfortunately hasn’t repeated), and stopped having the feeding problems he had been. Over time, his level of pain seemed to head back up yet was still better than when he is off it.
His perscription was increased from 24mg to 40mg about six weeks ago. Again, after a few days he had a day where he took three naps (doesn’t usually get more than one interrupted in a few minutes by reflux). He started being dramatically more cheerful during day time too but by the time six weeks has past, he is getting more and more fussy again. It really seems like it works better for him initially and than it does as time passes. Anyone else have this problem?
Thanks,
Kara

Inactiveare you using the 20 mg or 40 mg packets? if you are using 40 mg packs then you need to use mylanta supreme if you are giving less than 3.5 mL at a time, because there isnt enough sodium bicarb in those packets.
have you seen the dosing recommendations at marci-kids.com?
some babies just require much higher doses of meds than others. my son is actually on a dose higher than the marci-kids.com site recommends for zegerid.

InactiveWe found that the PPIs were somewhat weight sensitive, so her dose would work for a month or so, and then she’d gain weight and it wouldn’t work as well. 30mg has worked well for her, and she’s been on that dose since 10 months old without going up.

InactiveWe are using the 20 mg packets. His weight is not going up (which is a problem in and of itself), and according the Marci Kids site the 24mg dose was high end for his weight so the 40mg is significantly higher than recommended.
I guess I just don’t understand the surge of doing so much better and then the progress diminishing even though there has been no weight gain. It really seems like his body is adjusting or something so that the medicine is not quite as effective. (He is still better than when off medication however.) I was just wondering if anyone else had experienced this.
Thanks for your reply’s.

InactiveDon’t know much about the less effective thing over time, but it does sound a bit strange re the working only temporarily. Have you put the question to the marci kids people?
I was wondering has he only been on Zeregrid? Some mothers describe one PPI as not doing much but another as being more effective. Alana didn’t tolerate losec well, but the Zoton (precacid) works OK. If he isn’t doing well on Zeregrid would it be worth trying another? If it is his body adjusting, maybe it wouldn’t to another? Then you get the pain of timing though.

InactiveWe’ve tried Prevacid twice and he’s definitely allergic to it. I don’t think there are any other choices approved for infants besides Prevacid or Omeprazole (zegerid, Prilosec etc.). Maybe I should be thinking about something that hasn’t been approved but it makes me a bit nervous to head in that direction.

InactiveI heard someone talking about nexium on the forum recently…maybe you could do a bit of an internet search on that? I have heard of it being used well in adults…as do prilosec and prevacid (the later of which isn’t approved in aus, although my GP prescribed it
). Someone can pipe up if I’m wrong but the first two haven’t really been extensively tested in peadiatrics anyway, but they have been approved, so maybe it isn’t such a risk trying something different?

InactiveJust to chime in… we have had a bit of the same thing around here. My son hasn’t been on Zegerid for as long as yours, but it does seem to work really well for the first few days and then go downhill from there. Right now, we are struggling because, while he seems to be okay acid wise, he has horrid stomach cramps and gas. Could this be causing your son’s pain too? Dr. Phillips suggested that we go to the 40 mg. packs instead and add the Mylanta to help with the taste and help with the gas. We just started this on Thursday night, so we haven’t seen much improvement yet, but I’m hopeful. Maybe this is an option??

Inactivefyi: mylanta supreme was the cause of my sons stomach pain.
