InactiveHi everyone and Happy New Year
.It’s been forever since I’ve visited the site as we finally finished our move from NYC to the berkshires of Mass. It’s gorgeous here and we love having more space and the grandmother’s around. However, downside is we’re in GI hell and Lauren’s (who was doing beautifully for the last 3 months on Elecare and 30mg of Prevacid) has been having major reflux flareups for the last week and a half. It doesn’t seem to be getting better-she’s arching, crying during feeds, nightwaking, screeching, etc. It’s so upsetting because really she was the easiest baby -eating a ton, sweet and smiley, and sleeping great. We saw a local ped GI and although he seemed nice and competent, he ticked us off with his spook tactics. I wanted to run by what he said with you all to get your thoughts and see if maybe we should go out to Boston to find someone instead.1-I asked him if this was just perhaps a typical 4-6 month peak. Afterall that’s when our son starting vomiting blood from reflux. He said yes, it peaks now but our case was rare and different because although reflux peaks, the pain shouldn’t (rather just the spit up). Damage to the esphogas after it was controlled for so long is not likely reflux but perhaps a bigger problem.2-He asked me if Ian had been tested for esoniphils through endoscopy. I told him yes and twice. Both times were negative. He said he suspects Ian has that because his reflux and mspi (and Lauren’s) are both so rare and severe it should not be getting this bad at this time. He said Ian probably came back negative because he was on Elecare but when he’s older, it will probably come back (even though Ian’s doing wonderful right now-eating everything and no reflux for over a year) when he’s older. He suspects Lauren could have this rare disorder of ee as well. This ticks us off! He doesn’t even know Ian and I asked him, if you think both kids coud have this ee disorder and it came back negative on both endo’s Ian had (during his vomiting blood period), wouldn’t it have showed up then if that was what was causing the reflux?? He says no because he was on elecare, but my argument is then why was he having increased reflux that you’re saying is almost impossible. Surely if ee was the reason, it would show it’s head then even on elecare. Why scare us! He kept saying he didn’t having the time to get into the whole world of ee right now…THEN..why bring that up.3-He said to add mylanta but either taking it 2 hours from the prevacid OR take it exactly at the same time as the prevacid because the ingredients in MYlanta actually aid with the absorption of prevacid. (I’ve never heard this before). Any one else? So, I guess my biggest question to you all is I know many of you have had reflux flareups at this time of 4-6 months. When you’ve seen your children have flareups were they accompanied by pain like Lauren or just spit up, vomiting, etc and pain seemed managed? He has me all freaked that Ian’s going to come out with this rare disorder some day.Thank you! I hope you all are happy and well. Now that we’re settled, I hope to become a bigger presence on the board again!January 3, 2008 at 10:58 am #46889Anonymous
InactiveMy son is 4.5 months and I think that he is peaking. The amount of spit up has dramatically increased. He does seem to be in more pain. I am not sure if he is just annoyed with the spit up or if it is more acidic. He fusses a little bit when he spits up even though he is on 30 mg prevacid. He did not do this at 3 months. Last night- for the first time ever- he woke up every 2 hours. I am not sure if the “peak” that people refer to means volume of spit up or acidity of the spit up. Seems to me that to a certain extent, they go hand in hand. Maybe go get a second opinion? I am no expert, but I hope that helps! Good luck.

InactiveThank you Katie and yes it helps to hear that.
One final thing about Mylanta that the gi mentioned as well. He said babies of this age can have up to 4 teaspoons a day. And, I believe he was talking about regular not cherry supreme…isn’t there something in the regular that is dangerous for infants? Doesn’t that seem like alot?January 3, 2008 at 3:21 pm #46896Anonymous
Inactivemylo(Ashlynn) peaked exactly at 6 months, but she did not have any pain issues.

InactiveIt’s the aluminum in regular that’s the issue. Our ped GI said that you would have to use a ridiculous amount before the aluminum became a problem. However, I have two friends that work in GI clinics and say that they’ve seen aluminum become an issue much earlier. So, we used the Cherry Supreme. Our ped said that you couldn’t overdose on it, you just have to watch out for diarrhea.
I’m really not sure of what to think about the new GI. Sorry that he scared you so much.I grew up in MA (closer to Boston). The Berkshires are a beautiful area.January 4, 2008 at 4:33 am #46916Anonymous
InactiveSorry, don’t know much about ee. But when Alana’s reflux fared it was pain (she was not a spitter)- thats why the meds need to be upped to control the pain (as we all know it usually does nothing for the spitting). I’d stick to your guns, afterall you are pretty much an expert- both in your kids and in reflux.

InactiveTHanks everyone. I just think he shouldn’t be making such scary observations when he doesn’t even know my son and I had a gi and allergist team who said he outgrew everything.
Therese-when you saw a pain increase from reflux was she on an elemental formula? Was it around this time period? Do you remember what dose of meds she was on? I ask because I suspect Lauren just needs a higher dose – she’s a huge baby at 4.5 months-she’s already on 30mg but weighs 17pounds! Also, she’s silent reflux as well. Also, I’m so glad to hear Alana’s doing so well!!January 4, 2008 at 10:53 am #46930Anonymous
InactiveTracy,
We saw a peak with both of our kids at the 4 month mark. Actually, that’s when we finally were able to diagnose what was going on with Claire. She was silent refluxer too, and the doc. just thought she was nursing so poorly and arching because of a supply issue. HA! Ben, however, was a HUGE spitter, and his also peaked around this time too. I didn’t really notice more spit up, but he was definitely more uncomfortable. We increased his Zegerid dose at the 4 month mark and haven’t had to do it since. (He had RSV at the time too, UGH!) Anyway, I was breastfeeding, so not on elemental formula, BUT I was on the elimination diet and his reflux still peaked at that time.
My theory is that ALL babies even non-refluxers start to spit more at that age because they are moving around more, becoming more active. Thus, for refluxers, the extra spit, is just too much for the previous meds. to control, pain wise that is. Don’t know if that makes sense.
Ben was also a BIG BOY! So much for the supply issues the doc. thought I had.All that to say, maybe it wouldn’t hurt to try to increase the Prevacid, or maybe add in some Zantac? As for the doc. that really stinks. I don’t think they realize how much eveything they say can really affect us. It seems odd to me that he suspects this for Ian when he is doing so well right now, eating anything and no reflux…Bensmama2008-01-04 10:56:51

InactiveTracey, Alana has also been a bigger baby, always at the 90% for weight. We were on 30mg of prevacid already so I added in the max dose of zantac (10mg per kilo) and that seemed to do the truck for us. Also she was on Pepti Junior (like alimneutm) at the time, luckily we never needed to go to neocate (although in Aus we are lucky in that it doesn’t cost anywhere near that in US).

InactiveJust wanted to add that my lo is 4.5 months and definately more uncomfortable now than a month ago. It’s a nightmare, but that’s another story.

InactiveThank you, thank you, thank you! I feel SO much better. I went through the same thing with Ian (also 97% at this age) and he got hospitalized because of reflux. I guess I should learn to trust my instincts and head out to Boston to get another doc. It’s not that I don’t think it could be possible that they may have problems later on, but I really think it’s a huge long shot. I don’t think he should be saying things so definitively like that! It’s irresponsible imo. I think he had a bit of a god complex.
You guys are the best! -
