InactiveHi,
I need some advice from you parent experts. I’ll try to be brief. My 4 month old has silent reflux that was diagnosed 2 months ago. He currently is on one 15 mg Prevacid solutab once a day. We tried giving 1/2 in the morning and 1/2 the evening but it takes the entire Solutab to keep him comfortable throughout the night. Any less and he wakes up at 4:00 a.m. crying and never gets back to sleep. And he was doing OK during the day so I figured at least he’d get a good night’s sleep.
So this regime has been working until 2 weeks ago. His reflux is flaring and he hardly naps at all during the day (literally only 20-30 minutes at a time). His g.i. says he’s on the maximum dose of Prevacid for his weight. I asked about adding Zantac but he wants me to try Gaviscon drops once a day, which I gather are just like Tums and won’t do much.
Should I go ahead and just do the Zantac? I have a prescription from my ped and I’ve tried it and it does seem to help him nap during the day. I know I have to give it 4 hours apart from the Prevacid. I wonder why my GI didn’t want me to use it?
I also have given up dairy several times but keep slipping up. The G.I. suggested giving up dairy and maybe even soy.
I’ve also heard some people say the Prevacid should be given by 5:30 p.m. because the acid pumps shut off. We give ours before bedtime (around 7:00-8:00). The only reference I can find to that 5:30 rule is Dr. Phillips from the MARCI-KIDS website (which is shutdown). Has anyone been told this by their doctor? For us, it seems like if we give the Prevacid too early at night, he wakes up even earlier.
I’m so exhausted from the lack of sleep and having to hold my baby all day to keep him happy. It’s not helping my hernia! And it’s so awful to see him so miserable.
Any thoughts? Clearly the GI isn’t going to be much help. But at least he’s better than my ped. She told me 7.5 mg was the maximum dose of Prevacid!
Thank you in advance for your wisdom and advice.

InactiveI would keep trying to push for the increased dose; in the meantime, still give the 15mg whenever works for you (we’ve done 5pm or 8pm & both have been fine… as long as it’s on an empty stomach… 2-3hrs after last feeding… then give med, then feed 30-60min later). I would also do the Zantac @ 8am, then again @ Noon (both within the 4hrs of PPI). I would also completely eliminate all dairy & soy (direct & hidden sources) if you think that’s a problem (based on mucous &/or blood in stool &/or stinky gas).
Hang in there!

InactiveThanks Erin. I appreciate the support.
I’ve been giving the Prevacid one hour after a meal, then waiting 30 minutes, then feeding. My little guy rarely goes more than 2 hours without eating. Oddly enough, one time he had an AWFUL night and it was a time he ate, then we ended up waiting 2 1/2 hours before giving him his Prevacid. But maybe that was a fluke.
I will definitely start up the Zantac again and eliminate soy long with dairy. Soy is in everything (soy lethicin). No blood in stool but mucus and stinky sometimes. He’s always congested too.
So you said give the Zantac at 8:00 a.m and noon. You wrote both within 4 hours of PPI but you meant at least 4 hours apart from PPI right? If I’m going to keep giving the Prevacid at night, should I space the Zantac more (maybe 8:00 a.m. and 2:00 p.m.)?
Thanks again. This is so hard when parents know more about this than the doctors!

InactiveHmm… another option, is to try to get your dr. to script Zegerid. We switched to this when my 3rd ds was born (& we had 2 dc on PPIs @ once) & eventually switched both boys to it b/c you don’t have to time it around meals (& b/c they couldn’t tolerate Prevacid solutabs). Also, when drs. prescribe the Zegerid, the pharmacy (in almost all cases) will give you 40mg/day regardless of the dr’s dose b/c of they way you compound it (so you can dose what you want).
Anyway, yes, soy is in everything… just try to eat as few processed/packaged things as possible since it’s in so many things.
Yes, I meant 4 hrs apart, sorry :). You can certainly do 8 & 2 – whatever works best for you & gets db the best relief/rest/naps.
Hang in there!
