InactiveHello everyone!
I just wanted to ask the pro’s here if I’ve figured out the correct dosage for my little guy. I’m horible with numbers, and I don’t know that I’m understanding this correctly.
Anyway, he’s 5 months old and weighs 19lbs. 5oz. I came up with a range of 43-86 mg per day. Right now he’s only getting 5 ml of Prilosec once per day and 1 ml. of Zantac twice per day. I’ve read and re-read the page on Marci-kids about this but can’t figure out how many doses per day he should get. I know it has something to do with how quickly a baby metabolizes the medication. I’m thinking that I really need to understand all this before going in to talk to the doc, so I’ll really appreciate any help I can get!
His apnea monitor came today and I feel so much better…he’s been choking like crazy the last couple of days and seems miserable. Thanks for all the help!

InactiveWhen I figured it, I came up with 11 mg’s three times per day.
19.5 lbs divided by 2.2 = 8.86 KG x 1.25 mg = 11.07 mg 3 times daily.
Marci-kids recommends 3 times daily dosing for babies under age two.
How many mg’s per ml is his Prilosec?

Keymasterdawn,
there are 2 issues here:
1) to figure out a ‘good’ dose of prilosec for your baby &
2) to make sure it’s being compounded correctly (there’s a link from my welcome/intro called ‘Prevacid 101’ and from there there’s all about how to get a PPI compounded- made into a liquid)
Marci-kids recommends higher doses than what I’m about to figure out for you (what I’m about to calculate for you is a ‘good’ dose- fairly conservative but a good starting place- I hope someone else chimes in here w/ the dosing from marci-kids!)
here’s my calculations-
19.5 divided by 2.2 gets you weight in kilos = 8.87 kilos
if you take the dose of 3mg per kilo (conservative), then you multiply:
3 x 8.87 = 26.6mgs per day, total for the day, of prilosec
Christine came up with more, but as stated above, what I’ve worked out for you is a conservative dose…there’s definitely room to increase!
Now, as to how much your son is getting now: go get your prilosec bottle and it should tell you how many mgs/ml so we can figure out how many mgs he’s getting a day. it will look like this ___mgs/ml
Next: zantac: zantac dose is 5-10mgs per kilo
8.87 kilos
8.87 x 5 = 44.35mgs per day
zantac liquid is 15mgs/ml, so 44.35 divided by 15 = 3mls per day of zantac on the LOW side, with room to go to
8.87 x 10 = 88.7 mgs per day
zantac liquid is 15mgs/ml, so 88.7 divided by 15 = 6mls per day of zantac on the HIGH side
& your baby is getting 2mls a day…

InactiveThank you, Christine and Laura! His Prilosec is 2mg/ml.
Obviously I didn’t understand the calculations at all… that makes my brain want to explode!
Lol! I’m going to have to print out this thread. Just curious – how did your doctors react to you making suggestions about dosing?Another issue I’m wondering about is the timing between doses of Zantac and Prilosec. I read somewhere that the two can’t be given together because the Zantac will make the Prilosec ineffective – I’ve seen that they should be given two hours apart but I’ve also seen 4 hours. Does anyone know which is correct?

Keymasterdawn,
I don’t think there is a ‘correct’ spacing b/t the PPI & the zantac:
2hrs, 4hrs, 5hrs…each doctor has a different opinion. Jonah’s ped GI said 2hrs and that’s what we did. From my 3 yrs here, I’ve seen people post up to 5hrs b/t the meds (from their ped GIs). So, it’s really up to you…
As for how to get the doctor to agree:
I always FAXED the doctor. Short & sweet. 1rst I THANKED him
. Then I stated current weight & dose. ___is currently 8.87kilos. He is getting 10mgs of Prilosec a day.
then the symptoms- how they’ve improved slightly but __ & __ remain. Then I state what dose COULD be. I just said (please feel free to copy my words & calculations:
“the dose for Prilosec (for infants) is 1- 3mgs/kilo [again, this is my conservative calculations from 3 yrs ago- this was considered ‘aggressive’ then] and, w/ __ weighing 8.87 kilos, his current dose is only 1.1mg/kilo. I am requesting that his dose can be increased to 3mgs per kilo, which is 25.6mgs per day. I appreciate you attention to this matter. then came more THANKS
.This worked bcse it appeared that I knew what I was talking about. You can also fax info from Marci-kids, etc. (I just stated facts like above, and never got an argument. If you do, then you can fax the marci-kids stuff, etc).
Here’s the issue: 2mg/ml means he gets 2mgs with every ml of liquid you give him.
You give him 5mls a day, which is 2 x 5 = 10mgs a day of Prilosec.
This is LOW, given how even at a conservative dose he could be getting 25.6mgs a day! and, according to the ‘latest’ research, he could be getting 33.21mgs per day. Personally, I would ‘aim’ for the 25.6mgs to start with and see if there’s improvment…
HTH (hope this helps)

InactiveIn regards to calculating the dose from the MARCI-Kids website, please use our new dosing calculator, found on the PPI Dosing Page of our site.
