InactiveHi! Please help if you can!
Here is my situation… My son, Silas, is 6 mos old and has reflux. He CONSTANTLY projectile vomits/spits up (probably 6-10x’s per hour). He is on ready-to-feed Nutramigen mixed with soy-free rice cereal and 15 mg of Prevacid (solutabs). He was prescribed Reglan by the GI Specialist and it actually made his vomiting worse and he was inconsolably crying. He has now been prescribed the Bethanicol in place of the Reglan and so far the results seem similar to what they were with the Reglan. My daughter (now 20 mos) also had reflux and did very well on a combination of Reglan and Prevacid and has since grown out of the reflux. Unfortunately, the Reglan has not worked for my son and I’m not comfortable with the risk factors associated with it for him.
And one more symptom, we actually hear his food coming back up and him trying to keep it down by swallowing… It’s so sad to listen to. Anyway, I’m at a loss. Any advice or medicine ideas that I could run by the GI Specialist would be greatly appreciated. I guess I’ll be calling them again tomorrow… Ugh–what to do…

InactiveI think his dosage sounds low. It sounds like he is still refluxing and reswallowing. I would make sure I was giving it properly meaning 2 hours after his last meal and follow up 30 minutes after giving it to him with a full meal. Then I would try giving it multiple times a day. Most of us had the most relief with 3x a day. How much does he weigh? Maybe you can get your doctor to let you do a trial of a higher dose and then if it helps you could keep him on it. I know prevacid doesn’t help with spitting up but his almost sounds like its refluxing up.

InactiveThank you so much. This website has been a lifesaver for me for months. I’ve waited until now to post and I really appreciate your responses. I noticed in the “groupie intro” that the Prevacid solutabs contain lactose. I think my son has a lactose and soy intolerance/sensitivity /allergy (I’m not quite sure of the difference). Could this be posing a problem? Just curious before I look into increasing the Prevacid dosage.
Any thoughts on Bethanicol? I can’t find much info. on it.
Also, I’ve seen people post about Mylanta… Any thoughts on this? What does this do that Prevacid doesn’t?
The poor guy is just getting worse and worse.

Keymasterthere’s more on this (the ingredients of the solutabs) in ‘Prevacid 101’ & about each form of prevacid (& prilosec)- this can be found from ‘groupie intro’ and also from the ‘treating gerd w/ ppis’ post that is stickied in ‘Medicines Forum’ (scroll down to the purple post)
as for mspi and the difference b/t allergy & intolerance, there is an excellent explanation on page 2 of the ‘main’ MSPI page – this is also linked to from Groupie Intro and also can be found stickied in the ‘MSPI Forum’ – from page 2, scroll down to the ‘google cache’ and you’ll find it

mylanta is a ‘quick fix’ – I tend to use it like the father in My Big Fat Greek wedding used Windex
, lol! It helps with gas, coats the throat before a feeding, puts out the fire of an acid attack…as for Bethanicol:
to do a search: there’s the ‘search’ button located in the upper right hand corner of the screen
you put the key word/s you’re searching for [I’d just try searching for ‘beth’ to start
]then you choose “Topic Subject” from the drop- down menu of “Search In”
then I like to choose “All Forums” from the drop- down menu of “Search Forum”
then I like to choose “Topic Start Date” from the drop- down menu of “Sort Results By” (this way I get all the posts from the past that might help me)

You know, has your son been tested for delayed gastric emptying (DGE)? That’s what the reglan & bethanicol are for, but if they aren’t helping, I wonder if it might be something else that causes him to spit up so much. If he does have DGE, you might want to try another medicine called Erythromycin. That’s what my son is on, and it has done wonders for him. It has decreased his spitting up dramatically.
Also, an increased dose of Prevacid (with 3 doses, if possible) might help too. Prevacid is usually just for the acid pain, but it also helped my son’s spitting up a little as well.Good luck! I hope you can find something to help your little guy soon!

I agree that his Prevacid dose is quite low. My son did not get relief from his reflux pain until he was on the dose recommended by the researchers at http://www.marci-kids.com which for him was 10 mg/3 times per day. Does he seem to be in pain? If so I’d defintely push for a higher dose. I wasn’t able to convince Myles doctor to put him on a high dose until I printed and shared the marci-kids information with her. That did finally sway her.
I really don’t know much about the motility agents but it doesn’t seem that the one he is on now is helping much. Perhaps his dose is too low, or maybe he’d do better on Erythromyocin.
Maybe another formula like Alimentum or Neocate?
How’s his weight?
I’m sorry to hear that he is spitting up so much. Myles was a big spitter but not as bad as your little guy, and it was so hard to deal with on a daily basis. I hope you find answers for him soon.

InactiveThanks! I feel so frustrated. The specialist says “Wow! This definitely isn’t normal spit up!” but then doesn’t give me an actual diagnosis beyond reflux. OK, so I know he has reflux but what is causing it?
As far as the difference between an allergy vs intolerance, I looked at the linked site and I couldn’t tell much of a difference. The symptoms seemed the same. ??? Maybe I’m missing something?
As far as the DGE, he hasn’t been tested… I wish they would start testing him rather than just saying “reflux” b/c I don’t know what the actual issue is–something like this or allergies/intolerance… ???

InactiveReflux can be caused by and/or aggravated by allergy/intolerance, so that’s why finding the right formula can be very helpful. But reflux is also a structural problem, an immature LES which eventually matures as the baby grows and develops.

InactiveI just wanted to add that Isaac never did well on Prevacid but did very well on protonix. His dose does seem pretty low to me too.

Everyone has given you great advice. You are the first person I’ve heard of who has a child that spits up as much as my son did. My first thoughts for you without going through my whole long drawn out story is: the dose is too low, should be given 3 times a day. Also, if you are worried about the lactose or hidden dairy in the prevacid there is a ppi medication called zegerid, which doesn’t have any dairy or soy ingrediants. You can administer it irregardless of mealtime and it was a lifesaver fo rmy son. It’s very expensive, so you want to look into your insurance coverage before you get too far into fighting for med changes, etc.
My son also had multiple protein intolerances. So far he’s tested negative to all food allergies– yet cannot tolerate many foods. We eliminated soy, dairy, beef, and finally gluten and wheat products. His main symptoms were incredible frequent spit ups, inability to sleep for longer than two hours (until about 18-19 mos of age after removing the final gluten and wheat in our scenario) very fitful sleeping when he did sleep– and surprisingly good days once the zegerid was a high enough dose. Good days and horible sleeping and nights translated for us into : uncomfortable but distracted during the day. Upper and lower gi discomfort from reflux and hiatal hernia along with lower gi trouble based on protein intolerances.
something else to think about is testing. I hope you start learning from Laura’s intro and can get some help. First priority would be approiately healthy dose of the ppi med. Good luck, I really understand where you are coming from!!
