The MARCI-kids web site doesn’t discuss Prilosec dosing, probably because it’s not FDA approved…but does anyone know if it’d be the same as Prevecid or the others? We go to the doctor tomorrow and I’d like to discuss increasing my son’s dosage to at least 3 times a day. He’s on the compounded Prilosec, 2.5 ml once a day, but I don’t feel that’s enough. I’m afraid they’ll say “OK, how about 1.5 ml 3 times a day” and I’ve read that anything under 3 ml is useless because of the stomach acid.

you’re dealing with two kinds of measurements…

the mls is how liquid is measured

the mgs is how solids, in this case, medicine, PPI, is measured

first you need to know how many mgs of the medicine youre dealing with

then, you figure out the mls…or vice versa

for example: from lori:

According to marci-kids research on this, unless each dose is going to be 7ml or more (which it rarely is at the doses we’re giving), then it needs to be made with 16.8 percent sodium bicarbonate. (If you are giving 7ml or more each time then you can do 8.4 percent.) The concentration should be somewhere between 2-4mg/ml so that each dose has at least 3.5ml to ensure enough buffer.

SO: let’s use 3mgs per kilo because off the top of my head, four years ago this was considered a ‘high’ dose

take your baby’s weight in lbs and divide by 2.2 to find out his weight in kilos. then take his weight in kilos and multiply it by 3. this brings you to the number of mgs per day totla of prilosec for your baby. THEN the pharmacist makes it into a compound (mls) using the directions above…

Carson is 22 pounds and 23 months old. He takes Prilosec twice a day. He takes 15 mg twice a day for a total of 30 mg per day. He has been on this for quite some time. I think since he was a year. Generic Prilosec doesn’t come in 15mg capsules, so we get the 20mg capsules and take 4 beads out. There are 18 beads in the capsule. I put the beads on a spoonful of rice cereal and push them down a bit and:

out-goes-the-pacifier – in-goes-the-medicine

when he is sleeping, I give him the meds (in the morning) right before I have to get him up

He is SO GOOD at taking the medicine now. I just give it to him on the spoon and he knows he has to take his *meddy*.

The generic is called Omniprizole (sp). It is only $10 month for us.

I never did trust the pharmacists to compound any medicine so I worked and worked at getting our kids to take the ingredients inside of the capsules.

I hope this helps. Good luck. Each kiddo is different in how they will tolerate the meds.

You have probably researched this site and others but wanted to pass on that we notice SO MUCh when Carson eats anything with milk/soy/oats, he is so bad. Nothing will touch the reflux, vomiting day and night, crabbiness, naughtiness, etc. He didn’t show any allergies but intolerances now. So, if your son doesn’t repond to the meds, he may have milk/soy intolerance. I hope not, but just to give you the heads up!

I cut out dairy from the beginning (my daughter, who doesn’t have reflux, had a milk intolerance as did I as a baby). I’ve since cut out soy (about a month ago.) I definitely see a difference without those items in my diet. What a pain in the behind, but it’s worth it in the end. Funny, his ped. told me NOT to do it saying that he’s not seen much evidence that it works. At our last appointment, when I told him I believed Cade had silent reflux, I said “it DOES matter, by the way, what I eat!” and he just kind of blew it off. It’s amazing how we can be made to feel like idiots when we’re really doing great things for our kids!

That is horrible when YOU are sacrificing and doing the right thing for your child and the doc blew it off. I only can believe that patients from now, he will have a problem child case and recommend what you did and say to himself – wow, she knew what she was talking about! Moms always know!! You are definately doing the best thing for your little one!!

About the dosing-

Take his weight in pounds and divide by 2.2 to get his weight in kg. Then (if you want to follow marci-kids dosing) use the chart below from the marci-kids site. That will give you the MG of meds he should be taking three times a day. If you’re not good at math, post back with his weight and his current dose, and the mg/ml his solution is made at (written on the bottle) and we’ll help you out.

Pediatric Dosing for Omeprazole
Dosing dependent on age & weight

Age	Dose per day (mg/kg/ dose)
< 3 months	1.5 mg/kg three times per day
3-6 months	1.25 mg/kg three times per day
7 months to 2 years	1 mg/kg three times per day
> 2 years	1 mg/kg two or three times per day
> 5 years	0.75 mg/kg two times per day

I can do the math, I just don’t really understand the whole thing…or maybe I just can’t do the math 🙂 14.1 lbs. as of today at 9 weeks. He obviously doesn’t have trouble gaining weight…

They raised his dosage, but only to what I was already giving him (more than what they prescribed). So I just raised it again myself. I’m giving him 2.5 ml in the morning and 3.5 ml in the evening (prescribed 2 and 2). He’s doing pretty well during the day, it’s the night that’s killing me.

ok someone correct ME if need be!!

14lbs is 6.4kilos

< 3 months

1.5 mg/kg three times per day

that’s 1.5 x 6.4 = 9.6mgs one time a day, so then total for the day is

9.6 x 3 = 28.8mgs a day total

2mg/ml

so 28.8 divided by 2 = 14.4mls total for the day

divided up however you want

over the course of the day into 3 doses

That looks right to me. I swear when I did the math it made sense, but I did it quickly in my head so who knows how I got that.

Wow…we’re way far from where we should be on dosing. Well, she said to call if I needed a refill and I’m not even halfway through the bottle, so maybe I’ll just up the dose and go from there. I tried explaining everything and how I felt his dose should be increased according to my research, but they still wanted to take it slow. I’ll give it a few days and then decide.

Don’t feel bad! By the time your babe is done with this, you will be a pro giving someone else advice!

When we up Carson’s dose, it does take a while for it to kick in. Sometimes I think it takes a week – even though he was on the exact thing – only a higher dose. I keep a journal with meds/amounts/appetite/activities/sleep times/ etc so that I can tell what works and what doesn’t. Then if you change meds or the dose, you will be able to look back and see that things are hopefully getting better! It helps when things go down the drain, also. You can look back to see what helped last time.

Good luck with the meds increase!

When we were first trying to figure out the meds I thought I would lose my mind with it all. Even though it’s not that complicated, I think the sleep deprivation stopped my brain from functioning properly! Good luck with the higher dose.
