InactiveHi- We saw the pedi GI for the first time today (different from the one my older son saw because he moved out of state). Brian is 2 months old and weighs 11 1/2 lbs (50th percentile for weight). He is currently on 15 mg Prevacid solutabs which we give in a single dose at nighttime 30 minutes b4 a feeding. We have also been giving him 1/2 tsp Maalox as needed. The GI said his dose is too high for his age/weight and also said he prefers the liquid compound over the solutabs (he said we need to have it made at the hospital pharmacy to be sure it is done correctly). He said to give Brian 1.5 ml of the Prevacid compound 2 x day instead of the Solutabs and give 1 ml of Mylanta Supreme right before each feeding. The dose of Prevacid is lower than his current dose. That doesn’t make sense to me since he is still having fussiness on the higher dose and sleeping terribly. Won’t this make it worse? I told him my older son was on 40 mg Nexium split into 3 daily doses and he almost fell out of his chair saying that was such a high dose for an infant. My older son saw the head of Pedi GI at a major Childrens hospital here in St Louis so I never questioned the dose at the time. Plus he did great once he was put on Nexium. Now I am worried that we overmedicated him. But I worry that we are going to be undermedicating Brian if we lower his dose like the GI is suggesting. Anyone know about the Prevacid solutabs vs. the liquid compound and have any thoughts on these doses? Thanks for any input!

InactiveKelly, sorry to hear that you’re having a hard time with the doc and your son’s meds. I don’t know too much about nexium, so I can’t comment on that point. The problem is that different doctors follow different dosing standards- some are lower, and some are more aggressive. Marci-kids seems to be the more aggressive that I’ve seen, but their doses also work, all other things being equal. I do believe that a properly made compound is better than a solutab, BUT, it’s nearly impossible from my experience to get it made properly. That’s why I tend to go for the solutabs over the compounds. The fact that your doc is suggesting 1.5ml twice a day, would make me suspicious that he doesn’t really know what makes a stable vs an unstable suspension. Marci-kids says that at least 3.5ml per dose is needed to ensure enough buffer. There’s a link stickied in the medications section about compounded pharmacy PPIs. I would follow the marci-kids directions if you’re going to go that route, but like I said, I’d probably try zegerid or stick with the solutabs personally. The compound never worked for us.

InactiveThanks for the input! My older son (now 3) was started on the Prevacid compound through his pedi when he was 2 months old and when we saw the pedi GI for the first time several weeks later he told us that the liquid suspension was “practically useless” and switched John to Nexium. With the Nexium, we opened the capsule and placed a third of the little beads inside on a baby spoon on top of a small amt of applesauce and fed it to him. Once the Nexium built up in his system, he did great and we never had any issues after that even with teething, illness etc. I realize now that our pedi GI at the time was very aggressive in terms of dosing (I had no clue about PPIs and dosing at the time) but for John it worked well and he stopped having discomfort, ate well, slept well, etc. That GI moved out of state but I got an appt with one of his colleagues but it’s not until the end of Aug which was the soonest I could get an appt. We have Brian’s two month checkup with the pedi tmrw so I am going to discuss this with him then and get his input.

InactivePersonally I would stick with the solutabs. They have worked great for us. We are on 15mg 2x/day. I would also think about giving it to him 2x a day 1/2 in the a.m .and the other 1/2 in the p.m and see if that helps any and if now I would find someone who is willing to up your dose to give him more. We started at 7.5/day and then up to 15mg/day and have been on 30mg/day for about a month now and things have been wonderful. My dh is a pharmacist and he was hesistant at first but now is all for whatever I research and push for…. I would stay with the solutab, try splitting the dose and go from there. GOod luck.

Keymastermaybe you could fax the dr that moved and THANK her/him for helping so much with your son & then tell your situation with your baby- perhaps s/he can push to get you an earlier appt. w/ her/his colleague…
