We’ve had issues with our 6 week old since 2weeks. He was dx with Reflux and was put on Prevacid 7.5 2x day from the start but after being on it for a whole week i noticed no difference at all . Went back to the ped who suggested seeing a ped GI. We started with Zantac .6 twice a day and Carafate .5 30 min before meals. He was doing so much better and we saw immediate improvement. After being on it for a week he went back to being miserable so they increased the dose to 1ml of both meds. Same thing again and he improved but started to get worse and worse and they just put him on Axid 1ml three times a day and continue the Carafate. This combo is just not working and he’s eating less and less and sleeping worse than ever. It’s been three days and my poor little man is in pain. What’s my next step? I feel like i’m running around in circles and he needs something stronger. Any input? I had high hopes for the Prevacid but it didn’t work for us in the beginning so what next.
I’m bringing this up first thing tomorrow with the GI.

from what I noticed with my dd, the PPI took about 2 weeks to really work–she needed some time to heal from the acid damage to the upper esp. area—maybe ask about the dosage of the prevacid with the GI tomorrow?

I would give the prevacid a good 14 days to see it’s full effects

there is a post called ‘Prevacid 101’ where you can read about the various forms of prevacid (also other PPIs)- to access this link, go to the Introduce Yourself Forum & click on Groupie Intro- there’s a LOT of info there so maybe bookmark it & come back to it later…

I would check to see that you’re administering it properly

I would also check http://www.marci-kids.com to check for the appropriate prevacid (PPI) dose

in the meantime, i woud also give zantac, bcse until the prevacid starts to work it’s as if your baby sin’t on any meds at all- give the zantac 4hrs seperate from the PPI. I would figure out 5mgs per kilo- this is ‘conservative’ for a ped GI dose- we can help you do the calculations if you need us to- to make sure you aren’t just taking a med dose from a stranger on the internet, ask you pharmacist to consult her/his ipharmacist- an electronic device w/ up to date dosing-

oral pediatric dosing of zantac:

Duodenal and gastric ulcer: children 1 month to 16 years: 2-4 mg/kg/day divided twice daily to a maximum dose of 300 mg/day

GERD and erosive esophagitis: children 1 month to 16 years: 5-10 mg/kg/day divided twice daily. GERD max dosage: 300 mg/day. erosive esophagitis max dosage: 600 mg/day

Thank you girls for the great input. I’ve already made an appt with the GI again for tomorrow morning first thing. Last night we had another horrible time with Daniel. His formula just keeps coming back up. We tried thickening but both Oatmeal and Rice made him very gassy and uncomfortable. I wonder if i should give this a go again. He’s on Alimentum and Dr. Browns bottles. Please wish us luck tomorrow.

Good luck tomorrow. Sorry that you’re having a hard time with your little one. If the prevacid didn’t work, my guess would either be that the dose wasn’t enough for him, or that it wasn’t prepared or administered correctly. You might want to check out http://www.marci-kids.com. They have lots of information on PPIs and some info on dosing. Their doses are very aggressive, but they do work. Also, what kind of prevacid were you giving him when you first tried it? Was it the solutab or the packets? If so, then it should be given on an empty stomach and followed up with a meal 30 minutes later. The empty stomach ensures that the med won’t be destroyed in the stomach, and the meal activates the drug. If you don’t time it this way, then it won’t work well. (The only exception to this rule is zegerid or a properly made compound.) The other possibility is a milk allergy. I know he’s on alimentum, but maybe he needs neocate? Does he have any symptoms of MSPI? Welcome to the site.

Back from the GI. We decided to try the Prevacid solutabs again 15 mg divided into two doses a day. This is the max dose he’s willing to go so i’m hoping it will work. Also carafate 4 times a day 30 min before a meal.
I’m a bit confused as far as what’s considered an empty stomach since he eats every 2.5 but mostly three hours. So that means if he has a bottle at 6:00 am I can give the prevacid at 8 and that would be fine? That would put me an hour before the next feeding. Do the two doses need to be spaced out at 12 hrs ?

How about the Carafate?? How long can i give it after the Prevacid dose for it to not mess with the PPI?

We were given the Prevacid Syringe and i’m hoping it works. I tried a million different ways before and he ended up spitting it all out.

If he feeds at 6 and 9, I’d try giving the prevacid around 8:30 if you can. Then I’d give the other dose before bed or dinner if you can manage that. With the carafate, I’ll have to do a search… I thought it was 3hrs apart from the PPI, but that’s just based on memory… it might be hard to time it all. The two PPI doses don’t need to be spaced 12 hours apart, so don’t worry about that. Good luck. Hopefully the dose works.

My son is on Prevacid and Carafate. He eats about every 3 hours so we give him the medicine 1/2 hour before he eats which is 2 1/2 hours after the last feeding. Our GI told us the Prevacid and Carafate have to be given an hour apart. We alternate medicines at each feeding so he gets Prevacid before his first feeding, Carafate before his second and third feedings, Prevacid again before his fourth feeding and Carafate again before his fifth feeding. Hope that helps.

Thank you so much girls. This board is the greatest and i’m glad to be a part of it. Now a non-reflux Q if you all can help.

Daniel has been falling asleep at the bottle after 2.5 oz. It is almost impossible to wake him up when on a normal non reflux episode he takes a whole 4 oz at a sitting. This happens all day long. Could the stage 1 nipple from Dr Browns be too slow for him? even when it goes to up to 3months? I’ve been tempted to try stage 2 for 3-6 months but i’m afraid he’ll choke on a faster flow. Is taking me over 30 min to get him to drink 2.5 oz.

Yesterday we had a very good day and he still fell asleep on ALL feedings.

My guess is that if it’s taking 30 minutes to drink 2.5 ounces, the nipple may be too slow for him. I’d try the stage two. You can always go back it the stage two is too fast for him. Good luck!

oh & when you get a chance could you put baby’s age and meds/formula/breastfeeding, etc in your signature? so many people have joined recently & its hard to keep track of all these little GERDlings– just go to the very top of this page, go to the upper right hand corner and click ‘Settings’ and then Edit Profile and go down to Signature. thanks!
