Just got my son’s Prilosec & I’m trying to figure out when to give it/how often. I have no confidence in my pharmacist, since she was unwilling to listen to my concerns about the compounding.

The bottle says 2.5ml once a day & it’s a 2mg/ml suspension. Nothing is on the paperwork about when to give it, but she told me about an hour before breakfast.

So I’m trying to figure out how his dosage compares to the marci-kids dosing recommendations. I’m also trying to figure out if it HAS to be given an hour before breakfast & if it should only really be given once a day.

I should mention, she said the compound was made using omeprazole tablets. Not sure if that’s how the compound is always made or not. I have a friend whose daughter is on Prilosec compound & she gives it WITH the meal, so I’m just trying to figure it all out. I’d rather NOT have to time it & give it before breakfast as his mornings start anywhere from 6-9am & he is a MONSTER when he’s hungry!

Please help, as I’d like to give him his first dose tomorrow!

a PROPERLY compounded PPI can be given at any time.

as for the dose, it says 2mgs per every 1ml you give…

your dose is 2.5mls so that’s:

2mgs + 2mgs + 1mg = 5mgs a day. that’s all

Well, she said it wasn’t made using any flavoring agents.

She followed this recipe:

So I can give it with a meal? & should I still just give once a day, or should I be splitting it up & giving .85 three times a day or something?

The ped said his reflux was mild to moderate. I have to agree. He’s a fairly happy spitter…he just seems uncomfortable during sleep & once in a while he seems uncomfy after eating.

I would give the 2.5mls 2x a day, lol…but I can’t tell you to do that…

so try it w/ a meal…

He was only given a one month prescription…so we’ll have to call the ped in a couple of weeks anyway, as the pharmacy has to get the prescrip & then will take a few days to order in the stuff to make it.

If we give 2.5mls 2x a day, I don’t know what I’d say to our ped when it was gone in 2 weeks instead of 4…like, how would I approach that? I guess we could start off giving him 2.5 once a day, see if it does anything, if not up it to 2.5 twice a day & if that shows more improvement, let the ped know that we upped it because it wasn’t doing anything & we referred to marci-kids & got info on dosing.

But I should just stick with 2.5 once a day if that’s all I’m giving him? or should I spread it out several times a day?

In comparison my LO is 10lbs 12oz and we give 13mg 3x per day but we’re on Prevacid. The prevacid dosing is slightly higher than an omeprazole dosing based on the Marci Kids calculator.

It’s also my understanding that due to the half life of the drug in an infant’s bloodstream a PPI actually needs to be given every 8 hours or so to be consistently effective.

Your compound is only 8.6 percent sodium bicarbonate and you’re giving each dose of 2.5 ml. According to marci-kids each dose needs to be at least 3.5ml to ensure enough buffer per dose, and if the dose is less than 7.5ml per dose it should be 16.8 percent sodium bicarb.

So, I don’t know what to tell you about empty stomach or not. In theory, a properly made compound can be given any time.

Do make sure to refrigerate!!! Also, shake REALLY REALLY well before giving. All the sediment on the bottom is the bicarb- you need to shake it all off of the bottle to dissolve it. Sometimes it’s hard to see if the bottle is amber coloured. Good luck.
