Inactivenoah has been on level 1 dr. brown nipples. for the past 2-3 weeks he has only been eating 2 oz at a time and then he passes out or he refuses the bottle by crying, arching his back or swatting at it with his hands! so the last 2 days we tried a level 2 nipple and he drank so much better. BUT, his reflux and gas have gotten so much worse. he did not sleep at all last night. and when he did sleep he was thrashing, moaning, crying etc.
i guess i am looking for some guidance as to what to do. he will be 11 weeks on friday, he weighs 9 lbs 11 oz.

InactiveI’m so sorry to hear you’re having a hard time! Can you remind me what reflux meds/formula he is on?

Inactivehe is on 7.5 mgs 1x day of prevacid solutab and 1.1ml of zantac 2x day. elecare formula.
switched back to the level 1 and while he was a happier baby yesterday, it was a struggle to get him to take more than 2 oz at a feeding. the gi doc is concerned that if he takes in less than 17 oz a day then it is not meeting his needs nutrionally. sometimes we have to do a dreamfeed to get past 17 oz.
the ped gi wants to do a ph probe and an impedence test. he talked about maybe putting him on reglan. i am dead set against reglan as i was on it and had side effects.
i am just so frustrated. the little guy gives a 3 hour stretch from about 7-10 and then he is up almost every 2 hours. 1 am sleep deprived as i deal with the late, late nights and my wife takes the 9-1am shift (she is suffering from PPD so she needs to get as much sleep as possible to recover).
we really just dont know what to do….

InactiveHis dose is really low…have you checked the dosing calculator at marci-kids? Are you making sure to give the solutab on an empty stomach followed by a meal 30-60 minutes later?
I totally agree to AVOID reglan. It has been black boxed by the FDA for it’s potential scary side effects. I would push for a higher dose of solutab or switch to a different form like zegerid 40mg packets, nexium, or you can use buffer babies mixed with prevacid capsules, prilosec, etc…I think if you can get the reflux under control, the feeding issues will improve greatly!August 19, 2010 at 7:29 pm #66951Anonymous
Inactivedoes anyone know if you can use the prevacid solutabs and the prevacid capsules at the same time? not sure if the ped gi will follow the marci kids suggestions. he kinda brushed them off saying they were too high, even though we presented the website and research.

KeymasterI’m sure you could.
if you go this route, you might want to try switching to bufferbabies. I would email dr philips (or maybe amy knows) if you can use both rx capsules and otc capsules to mix with the bufferbabies.
this way you would not have to time some doses around meals ( as you do with the solutabs).August 19, 2010 at 7:39 pm #66953Anonymous
InactiveYou can definitely use both the solutabs AND the OTC capsules to get a high enough dose. Just make sure you open the capsule and sprinkle it into a little something acidic like applesauce, juice, or a little bit of pedialyte and dose it on an empty stomach followed by a meal 30-60 min later like you do with the solutabs. Make sure your last dose of the day is NO LATER than 5:30 pm. The delayed release PPIs (like prevacid) MUST be given before then or they won’t be effective at all.
You can also do buffer babies like Laura mentioned…but you cannot do BB with solutabs. Only Prevacid capsules, Prilosec OTC, generic omeprazole tablets, or rx omep capsules…and some others as well. BB is nice because you don’t have to time the meds around meals. There is more information about buffer babies at http://www.refluxsolutions.comAugust 24, 2010 at 10:03 am #66961Anonymous
InactiveHi, Amy, just want to clarify. Do you mean that if we give the baby prevacid beads from the capsule with applesauce, the last dose must be given before 5:30PM? But if we give the prevacid with bufferbaby, we can give at any time in a day? Thank you.

InactiveYes, that is exactly right. Buffer Babies, Caracream, and Zegerid can be given ANYTIME because they are immediate release.
Prevacid is delayed release and must be given before 5:30.August 24, 2010 at 11:47 am #66963Anonymous
InactiveThanks a lot Amy. Just in case Prevacid did not work and we have to shift to Zegerid, can we buy Zegerid OTC and give to baby directly or we have to mix Zegerid OTC with Caracream?
One more question, my daughter’s Prevacid dose should be 9mgx3 according to Marci, but a Prevacid capsule is 15mg, after I open one capsule, what can I do with the remaining 6mg? Can I store and use it for the next dose? Thanks a bunch.

InactiveYou need the Caracream for OTC zegerid because it tastes horrific. Dr. Phillips was talking about why you need MCS for Zegerid rx and Caracream for Zegerid OTC and this is what he said:
The Mylanta Supreme with the Zegerid gives you the benefit of three different buffers with the Zegerid packet 40mg. You see, the Zegerid packet as well as the Zegerid capsules contain sodium bicarbonate as the buffer. Sodium bicarbonate is a great and fast acting buffer, however, it can cause gas as it neutralizes the acid. So, when you add MCS (or Tums) to a Zegerid packet 40mg or CC to Zegerid 20mg capsules – you get a blend of buffers so sodium bicarbonate gets help and the amount of the bicarb is lowered = less gas.
Does that make sense? Also, when splitting a Prevacid OTC capsule it is virtually impossible to split it exactly and also impossible to overdose…I believe Marci recommends just rounding up to the nearest whole value (30mg) and giving her half and half. The difference between 27mg and 30mg is very minute and it would make your life easier. 🙂August 24, 2010 at 12:25 pm #66965Anonymous
InactiveGot it. Thank you so very much for your prompt responses. You are wonderful!
