InactiveI’m trying desperately not to panic.
Reading some of your other posts, I saw that many of you continued on Zantac while starting Zegerid, just to give the new medication a good two weeks to begin working. I didn’t know that until earlier this week when I was looking up why in the world her reflux symptoms were getting much worse.
I did start giving her Zantac again, trying to space it out at least 2 hours in between, but I’m not noticing much improvement. As a matter of fact, we’re on day 14 now, and her old symptoms seem to be worse than they had been on the old method of omeprazole (grinding up hard pills, mixing with applesauce, and choking it down her). She’s waking up arching her back and screaming in pain, and it’s happening while trying to put her to sleep, too. It’s just the way it used to be while she was just on Zantac.

Someone, please help me. There has to be a way out of this. If Zegerid can’t help her, I’m going to want to jump out a window.

InactiveI understand your fear and frustration, and I hope your daughter finds some relief quickly.
What is the dose she is getting and how much does she weigh?
Are you using the 20mg or 40 mg packets? You have to use the right size packet depending on the dose being given.
Have you looked at the PPI dosing information at marci-kids.com? Perhaps her dose is not high enough. Also, they have very good instructions on how to prepare the medication. Maybe you are doing something wrong in the preparation. Here’s the link http://www.marci-kids.com/dosing.html
She’s may have outgrown her dose of Zantac and so it isn’t helping her while waiting for he Zegerid to work. Zantac is very weight specific and babies quickly outgrow their dose.
Many moms use Mylanta Cherry Supreme and say it helps. You can do a search to find the correct dose. (Sorry, I don’t know the dose, but I’ve read here that too much will only cause loose stools).
I’m sorry you are having such a hard time and I hope you can find some answers quickly.

KeymasterI’m so sorry you’re still having so much trouble! we never used zegerid (didn’t exist) but we did use caracream (& LOVED it), which was created by the same people…
zantac dose (from here, among other links: https://www.infantreflux.org/forum/forum_posts.asp?TID=836)
is 5-10mg per kilo a day- 5mg per kilo a day on the ‘low side’ & 10mg per kilo a day on the higher side…

InactiveActually, the doctor who helped us out is giving us a high dose. Emily is about 7.5 kg and 6.5 months old, so he basically prescribed the 40mg packets and divided it into 13mg 3x/day. It’s definitely more than the 1.25mg/kg recommended, but he said what Joel says–omeprazole is so cell-specific that it’s nearly impossible to overdose.
Meanwhile, I suppose Emily is on not nearly enough Zantac.
I guess I’m just worried that it’s two weeks in and things seem worse than they were before, when she was just on the other type of omeprazole (the one that shouldn’t be crushed).

InactiveThat’s good that she’s on a high enough dose. You are lucky to have such an elightened doctor.
The reason I asked about her dose was to see if you were using the right size packet for her dose, which according to Marci-kids, you may not be. They say that you should use the 40mg packets for doses over 14mg, and the 20mg packets for doses under 14mg. You are preparing 13.3 mg doses. I don’t know if that small difference would matter, but maybe look into that.
Also, are you following Marci-kids directions for preparing the medication? Sometimes doctors don’t give the same directions. I think Marci-kids is probably the best source for this information since it is Dr. Phillips who invented this drug.
I wish I had some more advice for you but I really don’t know why Zegerid packets would not work when the omeprazole you used before did. Hoperully Joel will see this and answer your post, or perhaps you can email him and see if he has any ideas.
I’m sorry I can’t be of more help to you. I sure know how hard it is to have a baby miserable with reflux.

InactiveIf you go to the MARCI-kids site, scroll to the bottom there is a link that says “ask us” or something like that. If you use that and email them your questions/concerns, Dr. Phillips will actually respond to you. I emailed him once and had a response within the hour!

InactiveHave you tried any other ppi’s other than zegerid? It may be the “golden cure” for some but it’s not the end all of reflux meds, and for a lot of people it is nowhere near the best choice…Aidan’s dr mentioned it to us, gave us the choice of that or prevacid, and I refused the zegerid…

InactiveBased on your advice, Kristen, I did go to the website and email them. I’ve already heard back from Dr Phillips asking for her date of birth. Let’s see if we get somewhere.
Our enlightened doctor is a very quirky family friend who doesn’t believe in bureaucracy. He read the information I sent him and already knew that PPI overdosing is nearly impossible, so he wasn’t worried about giving her a higher-than-normal dose. He was very sympathetic because he now realizes his firstborn had GERD, but it was just unknown back when he was in residency. He definitely wanted to help another kid in the same situation.
Is it possible that Zegerid won’t work for her? It scares me to think of that, since this was my Holy Grail. 🙁

InactiveZegerid has worked great for us. Our daughter has been on it since early March and within 2 weeks, her symptoms were basically gone. We have increased her dose 2 times since then and she is now on 4.5 ml 3 times each day. We give it to her at 6:00 am (when I have to wake her up so I can get to work on time), 12:30 pm or so, and again around 7:00 pm. She will be 4 months old next week. She is gaining weight well and is basically a very happy baby. We give the Zegerid to her in 1 oz of formula, then have another bottle made up for the remainder of her feeding. We use the 20 mg packets and I mix up 2 packets with 13 ml of water in a large syringe and I put individual doses in small syringes – works great, especially to take to day care.
Hope things work out well for you. I know that Dr. Phillips will help give you some direction – he was a great help to us, as well as Joel (I miss reading Joel’s input on this board – hope he gets back on soon

I can’t imagine that Zegerid wouldn’t work for her. Hopefully it’s just some quirk in how it is being mixed or administered and Dr. Phillips can help you with that.

InactiveDr Phillips emailed back and forth with me yesterday afternoon, and it seems we may have an answer. I was giving her 6.5ml in some of her doses, which isn’t high enough (must be 7ml) to provide enough buffer to prevent the drug from being destroyed by stomach acid.
I don’t know if it’s enough of a difference because 2 of the 3 daily doses given were 7ml, but it’s a hope. He has me giving 6.5ml of the solution and then mixing 3.5ml of Maalox or Mylanta into the 10ml syringe.
I hope to God this works. Last night was utter hell, and I can’t keep going through this and watching my baby go through it, too.

Zegerid didn’t work for us…I really think Ava reacted to the flavoring…it
wasn’t the reflux but gas that kept her up at night etc!
We stuck with it for 4 weeks
We had much better luck with unflavored compounded prilosec
hope things improve!

InactiveI wondering…if Zegerid doesn’t work, will Prilosec? I’m thinking know. We are only on day 7 or 8 of Zegerid and I’ve seen no improvement in either twin. My ped. wrote the script for Zegerid after I saw the ped GI and she wrote it for prilosec. We go back to the ped GI next week. If the Zegerid doesn’t work by than I’m worried she will make us try the prilosec. My thoughts is that it’s basically the same thing, just a different coating, so if one doesn’t work, the other won’t work.
So, if Zegerid/prilosec doesn’t work, what else is there? We already did Prevacid (powder and solutabs), Zantac, and Reglan.
