InactiveI noticed that a few people were saying that they are on the max dosage of prevacid, 30mg, why is that considered to be the max dosage?
Has anyone ever used more than 30 mg?August 16, 2007 at 8:56 am #41180Anonymous
InactiveThere is no absolute “max dosage” of any PPI. Different doctors follow different dosing guidelines- there are the guidelines set by the North American Society of Gastroenterology, Hepatology, and Nutrition (which are on the conservative side), the guidelines set by individual hospitals and clinics, marci-kids (which is on the very high side), and others. Each dose is weight, age, and symptom dependent (i.e. GER vs. GERD vs. ulcers or EE). So when people say that their child is on the “max dose” of a PPI, they probably mean that their doctor has told them that they can’t go any higher. Many doctors don’t like to go higher than 30mg, often because this is considered a daily adult dose. But that doesn’t mean that it’s an absolute maximum. s&h’s mum2007-08-16 08:58:09

Inactive… oh yeah… and we have gone higher than 30mg. 30mg was the highest that we were ever prescribed, and it was a challenge getting that prescription, but we have gone as high as about 42mg per day.

InactiveWhen you were giving prevacid, were you giving it 2 or 3 times a day? Which way is better?
What was your schedule for giving it??How long did it last in your child’s system until it was fully metabolized???She doesn’t have trouble with her first bottle from the whole night because her stomach is fully emptied from the night, so her med schedule is like this6- bottle9:30-16 mg prevacid, 2 ml of erythro,10:15-feeding11:30-bottle3:00prevacid 16 mg, 2 ml eryth3:30-feeding5:30-bottle8-bottleAlso, how do you get in both solids and bottles in the day, it is much harder due to her dge.August 22, 2007 at 8:23 am #41506Anonymous
InactiveI know people out there are using prevacid, looking for advice please………..
Thank YouAugust 22, 2007 at 12:27 pm #41513Anonymous
InactiveMyles was on 30 mgs which was split into 10 mgs three times daily. I used to give him his first dose before his first feeding so that made it easier to squeeze in three doses.
6:30 am Prevacid
7am bottle
10 am solids
12 noon bottle
2:30 pm Prevacid
3:00 pm bottle
5:00 pm solids
7:30 pm Prevacid
8:00 bottle
Dr. Phillips at http://www.marci-kids.com does recommend three doses of Prevacid a day because babies metabolize these meds so much faster than adults.I’m not sure if or why 30 mgs is considered the max dose, but it probably has something to do with the fact that as babies get older their metabolism slows down and so the mgs per kilo ratio decreases as they grow. In any case according to marci-kids these meds are non-toxic so you really don’t have to worry about over dose. What problems are you having with Adi?What form of Prevacid are you using? (If I remember correctly it’s capsules w/ caracream).August 22, 2007 at 1:00 pm #41516Anonymous
InactiveI am using capsules w caracream which is working, but the worst feed is the 10 one in the am. the 6 am one is fine and that is before she even got anymedicine yet????
Makes me wonder…….August 22, 2007 at 1:47 pm #41517Anonymous
InactiveThat actually does make sense to me. She might do better on three doses, with one being right before bed. Even though she’s getting a dose right before her 10 am feeding, she could still be uncomfortable from such a long space of time without any meds. Prevacid works best with continuous dosing. It doesn’t work as well as the H2 Blockers (like Zantac) do in giving immediate relief.
I’d try giving her three 10-12 mgs doses instead of two 16 mgs doses. I’d try that for two weeks and then if she’s still having difficulites I’d ask to use an H2 blocker in additon to the Prevacid. Some babies will have break through acid at night or in the early am even when they are on an appropriate dose of Prevacid, and adding an H2 Blocker before before bed often solves this problem. If you use both you do have to space the H2 Blocker 4 hours apart from the Prevacid, though.I’d try the three doses of Prevacid first. That’s what marci-kids recommends and lots of babies here have done well on that (including my little guy).August 22, 2007 at 3:57 pm #41522Anonymous
InactiveThank you,
Yes the prevacid is helping but do you see your child being perfect with it or just an improvement???????Will she ever be without reflux or just an improvement???Please advise…August 22, 2007 at 4:15 pm #41523Anonymous
InactiveMy son’s pain seemed to be gone once we got his Prevacid dose correct. He still refluxed but it didn’t hurt him anymore.

InactiveI think it really depends on the child and how severe their reflux is, and what symptoms you are hoping to get rid of with the PPI. A good dose should control the pain. It might not stop the feeding problems completely if they’ve been there for a long time. Some babies act completely better while they’re on a good dose, others just see an improvement until they outgrow it. Is MSPI a possibility? That could be the missing piece of the puzzle.
