Inactivehi All,
Up until now the prevacid has been wonderful, and it’s worked out well for the most part timing-wise with Dagney’s feedings. Well, she seems to be weaning herself from one of her bottles, the 10:30-ish (am) bottle, then she’s really hungry at the 1:30-ish bottle, and sucks the whole thing down. This makes me think that I need to switch her to 4 from 5 bottles/day, but that would upset the Prevacid schedule. Right now she eats around: 7:15 (6-8 ounces), 10:45 (1-3 ounces), 1:30 (7-8 ounces), 4:30 (6 ounces), 7:15 (6-8 ounces). She gets her prevacid 20-30 mins before the 7:15, 1:30, & 7:15 bottles. If I move her to 4 bottles/day, then I figure they would be at about: 7:15, 11:15/30, 3:30, and 7:15. This would upset the Prevacid schedule, though, the 2nd dose would not be on an empty stomach and would not be followed 20-30 mins later by a bottle.
Has anyone else faced this issue? WHat have you done to deal with it? Is it ok that she gets it then? She’s starting solids now, and has oatmeal at the firrst & 4-ish bottles, and will be starting veggies next week, so she never really has a true “empty” tummy anyway, or won’t to my estimation. Knowing that the solutabs works well for her makes me hesitant to switch her to any other medication for her reflux, but if there’s something that would work just as well, but better for her schedule, I’d be willing to try it…
Thanks!!

InactiveI would think that Dagney is totally capable of going 4 hours between feedings at her age.. especially when you are adding some solids too. Ben has been for quite some time now, and sometimes he goes even longer.. depending on how well he naps! As far as the timing thing goes…. I don’t do Prevacid, but what I would do is maybe move her second dose before the late afternoon feeding (about 2:30 or 3)?? You could also give it to her 2 hours AFTER she eats. I think I’ve read somewhere that it works that way too.

Inactiveprevacid is much more effective when the med is given 30-60 min before the meal, not 20- 30. here is the info:
How Delayed Release PPIs
WorkIn
their pure and unprotected form, PPIs are very vulnerable to degradation when
exposed directly to the acid in the stomach. For this reason, DR PPIs are given
an enteric coating. This protective
coating is specifically designed not to dissolve while the drug is in the acidic
fluids in the stomach, but instead it dissolves when the tablet or granules
reach the higher pH (less acidic) environment of the duodenum (the first
section of the small intestine). Here the coating dissolves and the drug is
absorbed into the blood stream, where it is carried to the parietal cells,
where it does its job of blocking proton pumps. This whole process takes 30
minutes to 1 hour.DR
PPIs should be given on an empty stomach because food in the stomach can both lower
the level of absorption and raise the pH in the stomach, causing the enteric
coating on the drug to dissolve, which then leaves the drug vulnerable to
exposure to pockets of acid in the stomach which could destroy the drug.DR
PPIs should be given 30 minutes to 1 hour before a full meal (6 oz or
more for infants). As I already mentioned, in order for PPIs to work their best
you want as many proton pumps to be in an active state as possible, and the
primary way of achieving this is by eating a full meal. The timing is important
because you want the meal to be activating lots of proton pumps right at the
same time that the drug is reaching peak levels in the blood stream.DR
PPIs should taken with water or sprinkled over an acidic food such as apple
sauce or yogurt.You
should never break or crumble a tablet form of DR PPI, as this will compromise
the protective enteric coating. (Prevacid Solutabs contain enteric-coated
granules within the tablet, and it is ok to break a Solutab in half, as this
should not harm the individual granules)You
should never give a DR with a non-acidic food (such as milk, formula or
cereal), as this will dissolve the enteric coating and leave the drug
unprotected.DR
PPIs should never be chewed or crushed, as this will compromise the protective
enteric coating.
so maybe you can try an hour before you feed her?nataliachick72007-3-7 15:29:57

InactiveMyles was on a very similar schedule for many months:
Prevacid 6:30 or 7 am, bottle 7 or 7:30 am
10ish he’d have oatmeal
12- 2:30 or 3:00 he’d nap, so
Prevacid at 2:30 or 3 pm, bottle at 3 or 3:30 pm
5 ish he’d have oatmeal again, and
Prevacid at 7:30 pm, bottle at 8 pm
I was worried that the second and third Prevacid doses were too close together, but it worked fine and that’s the only way I could fit them into his schedule.
