InactiveHi, I’m a newbie to this board. DS is 7wk old and has reflux. In the
beginning, I noticed that he’s very squirmy during nursing. At 3-4wks,
he started having episodes where he would scream and cry like he was in
pain during nursing. He would have wet burps, not always spit up, but
his burps seem to be painful. He would still be in pain after burping,
and would stop nursing. He sleeps very poorly. I can’t put him down
for more than a few minutes on his back. During the day, he’s held and
frequently spent the day in the sling which is the only way he would
sleep. At night, he initially would wake up every hr or hr and a half
crying and I would nurse him to sleep. Around 4wks, he started to wake up
and cry every 30-40min. I’d nurse him back to sleep. Then I realized
that he wasn’t actually hungry, he was nursing for comfort because he
would nurse for a min or two and fall back to sleep. He could go 3hrs
without eating during the day if I hold him. Even when I hold him to
sleep, he has bursts of crying. I noticed after he sleeps for a little while, he
would writhe in pain, then he would be swallowing, burping, or
sometimes spitting up (30min to an hr after last feeding even after we
hold him up for 30min), and subsequently wake up crying.We started zantac after his 1mo apt. Amazingly, after 3 doses, he had
2 2.5hr stretch – before that he had never slept that long without
being held. However, a few days later, we are back to waking up after
less than an hr. I tried swaddling, swinging, bouncer, elevating his
crib, even letting him sleeping on his tummy. Nothing seems to make
any difference. We switched to prilosec last Tuesday. After a wk of
prilosec, everything was the same. We had a GI apt 2 days ago and
discovered that his dose was too low. He is 10lb13oz, was on 0.7cc of
prilosec at 2mg/cc. This was increased to 2cc bid. We are on day 3
and no change. His sleep seems to have a pattern: he would have 1
stretch close to 2h, the next stretch lasts about an hr, then
every 20-40min he wakes up crying until he wakes up in the morning. We
are exhausted! I’m going back to work next Friday and I don’t know how
I’m going to stay awake. We are getting a nanny, but I’m worried how the nanny can take care of him like that. He is still having pain drinking. It
doesn’t happen with every feeding but some days worse than the other.
The last 2 days seem to be worse, he’s not even interested in nursing. He only had one
good nursing session yesterday. The rest he only nursed for a min or
two then stopped. I was so engorged that I had to pump a few times.
He has not had problem gaing weigh before, but I’m worried that he will if he keeps having pain and refusing to nurse. We are also using maalox as needed but don’t see any improvement.Sorry this is so long. Would someone please let me know what else we
should do and try to help his pain and help him and us sleep better?

InactiveLi.. Welcome. I think you have come to the right place. There are several things that come to mind…
1. Check out http://www.marci-kids.com They have current research and info. regarding reflux meds. Most babies are underdosed because they metabolize the meds much more quickly than adults, but the doctors don’t take this into account. They also suggest giving the medicine up to three times a day, for the same reason.2. compounded PPI’s are notoriously unstable. Check out the stickied post in the Medicines forum.3. Could you have OAL? I did/do and this made nursing SO much more difficult because my milk would spray out and choke him and make his reflux even worse. I have several ideas to help this if you do.4. There is a possibility that he is reacting to something you are eating. Usually intolerances are to milk/soy… however, before cutting this out of your diet, I would first work on getting him on a decent dose of a PPI.It certainly sounds like he is in a lot of pain, so I would check out marci kids, call your doctor back, and fight to get him medicated properly. Best of luck to you. I know how difficult it is. I’ve had two refluxers myself, and it can be so exhausting and disheartening, especially when you are trying to nurse them. I have some ideas, too
:1) until you get the meds ‘figured out’ I would give the zantac also- make sure to give it 4hrs seperate from the prilsosec & figure out 10mgs per kilo for the dose (discussed here: Zantac dosing- my/other’s question: https://www.infantreflux.org/forum/forum_posts.asp?TID=9539&PID=82914#82914)he weighs 4.9 kilos4.9 x 10 = 49mgs total of zantac for the daydivided by 15 to get mls (your bottle should read 15mgs/ml) =3.3mls total, for the day, of zantac, divided up into 2 or 3 doses however you see fit (ex: 1.1mls 3x a day or, 1.65 2x a day or, 2.3mls at night and 1ml in the morning, etc)2) read through this:Groupie Intro: my intro: https://www.infantreflux.org/forum/forum_posts.asp?TID=853&PN=1&TPN=1

InactiveHi. Sorry to hear that you’re having a hard time. I really suspect that the prilosec isn’t doing the trick either because of the dose or b/c of how it’s made. It can’t have any flavourings whatsoever, and needs to be made with 16.8 percent sodium bicarbonate, with a concentration between 2-4mg/ml, and each dose needs to be at least 3.5ml to ensure enough buffer per dose. I don’t know how many mls 2cc is equal to b/c I’ve never heard it measured that way, but my guess is that it’s on the low side. A properly made prilosec suspension tastes really yucky. You might want to look into zegerid, which is a correctly made suspension that doesn’t need to be compounded. Check out the links as suggested by Naomi. Also, some babies need higher doses of PPI to get comfortable. We tried all kinds of doses, and nothing helped her until we were on at least 30mg. Good luck sorting things out. Have you considered MSPI/elimination diet?

The link to Marci-kids is very helpful. I double-checked our prilosec dosage and DS is getting the right dose at 1.5mg/kg except we are giving it to him 2x a day instead of 3x a day. I’m going to call the pharmacy tomorrow to find how they made the compound prilosec. I don’t think it has any flavor in it because it tasted horrible when I tried it, but I’ll make sure. I’m going to give prilosec another week since we basically wasted a week taking the wrong dose. I will ask the GI doc about zegerid next week since it’s weekend already.
I’m not sure if I have OAL (is it overactive letdown?). DS also is in pain when he drinks either expressed BM or formula from a bottle.
Thank you Laura. I learned a lot from your posts. I calculated the zantac dose that DS was getting. His wt was a little lower when it was originally prescribed. He was basically getting 7mg/kg in the beginning which isn’t too low. I’m definitely consider giving him zantac again 4-5hr after prilosec.
