So my daughter was diagnosed with acid reflux at about 1 month old. We put her on Zantac andAlimentum. It helped for a week or two. She started screaming again. We took her back and they went up on her dosage twice since then. after it stopped working again, they switcher her to prevacid tablets. 2 Weeks into it my daughter is still screaming and arching her back. I called the doctor back today and they acted like iI was crazy. I don’t really know what else to do. She cries and I cry. Any suggestions? & are there test they can run to help diagnose how bad her reflux is??

ok well first of all how much does she weigh?
I can help calculate zantac dose
as for the prevacid, I can help with that, too – just tell me what form she’s taking, sounds like prevacid solutabs?
how are poops?
as for tests, well yes but first you give the meds a shot and then when things don’t improve- but the tests are invasive & if you give the proper meds in the proper way, at the proper dose, you should start to see some improvement – there IS hope

I’m not trying to freak you out! just better be safe than sorry – the black flakes most likely were from the prevacid- sometimes it can look like purple flecks- but anyway, just check (yeah, another thing to do, sorry)

On to medicine:
PLEASe research all you can! don’t ‘just’ take my word on things! I DO know a lot, ha! but YOU’re the mama & I am not a medical professional, I’m just a mom who had a baby w/ severe GERD 10 yrs ago (!) and can’t stop helping others because unfortunatly not much seems to have changed over this long time so many drs are still so_____ (fill in the blank)…
my guess is that her zantac dose can be increased – let’s see:

you say your baby is 10lbs 14oz

that’s 5.4 kilos
maximum zantac dose is 10mgs per kilo
so: 10 x 5.4 = 54
that’s 54 mgs, total for the day of zantac that your baby can have
your bottle of liquid zantac says 15mgs/ml
this means that for every 1ml of liquid you give, there’s 15mgs of the medicine zantac in that 1ml of liquid
so: 54 divided by 15 = 3.6
that’s 3.6 mls of liquid zantac that she can have in a 24 hr period
divide this into two or three doses & the doses do NOT have to be equal, so if evening/night is the worst you can give a higher portion then
now, onto the solutabs:
first of all you need to give them on an empty stomach, followed by a ‘meal’ 30 minutes later. look at the stickied’ posts. This means a post that is ‘kept’ at the top of the forum, so when you go to the Medicines Forum and look at the top few posts, you will see there are important, ‘stickied’ posts at the top, for example: PREVACID 101 (I realize that there’s gobblety gook on this very important post- arg. I’ll fix it soon…) – but there’s other ones, too, which explain the PPIs and why you need to give on an empty stomach followed by a ‘meal’ 30 minutes later…

PPI Dosing
Proton Pump Inhibitors (PPIs) are the safest, most effective medications available for treating acid reflux-related disorders in children and infants. They include:
omeprazole, found in Zegerid®, Prilosec®, and generic form
lansoprazole, found in Prevacid®
esomeprazole, found in Nexium®
Each of the above listed PPIs are currently approved by the FDA for pediatric use for children over one year old.
For more information on the literature pertaining to pediatric dosing of PPIs, see Research.
The following are dosing recommendations for pediatric patients and are based upon the research and usage of the MARCI-Kids team at the University of Missouri. In some children, a higher or a lower dose is required to control symptoms. These recommendations are designed to be a starting place to assist your physician in choosing the right dose for your child.
Pediatric Dosing for Lansoprazole (Prevacid®)
Dosing dependent on age & weight
Age Dose per day (mg/kg/dose)
< 3 months 1.5 to 1.75 mg/kg three times per day
3-6 months 1.25 to 1.5 mg/kg three times per day
7 months to 2 years 1 to 1.25 mg/kg three times per day
> 2 years 1 mg/kg two or three times per day
> 5 years 0.75 to 1 mg/kg two times per day
Pediatric Dosing for Omeprazole (Zegerid®, Prilosec®, and generic form) and Esomeprazole (Nexium®)
Dosing dependent on age & weight

so let’s calculate your baby’s dose (see above) as:
1.5 mg/kg three times per day
that’s 1.5 x her weight in kilos, which is 5.4 kilos
1.5 x 5.4 = 8.1 mgs this is one dose
you give three of these doses, so that’s
8.1 x 3 = 24.3 mgs total for the day of prevacid
your solutabs are in 15 mgs so break them in half – try 3 doses of 1/2 of a 15mg solutab – this works out to be less but see if it helps, along w/ the zantac (it’s a start & you know you can go a bit higher)

The nurse I talk to when I call already thinks I’m a crazy lady!Lol. I GuessI am with a screaming 3 Month old! My LO only gets 1/2 a solutab once a day That’s a huge difference.

If you have Zantac left, start giving it, too, spaced 4 hrs from the solutab – it’s a start…see how that goes…I know I sound like a crazy drug pusher, but this is what I did w/ my son. The reflux meds are really very safe, especialLy the Zantac
In the meantime, TELL your ped you need a ped GI – see if there are any listed near you (look for stickied post in the dr forum)
Me? Knowing what I know, I’d also increase prevacid on my own, but I canNOT tell you to do this! If it works, well…then slowly cut it down to what the dr told you & see how that goes- if you see that proper dosing works, well there’s your proof…again….you’re the mama. Read all you can…find a dr who will rx what you tell her/him to…

I found this site, got my baby properly medicated & I can’t leave here, 10 yrs later.
My son will be 10 next month & is healthy & intelligent (&handsome!).
He doesn’t remember a thing!

June 19, 2013 at 8:12 pm #72387AnonymousInactiveOh my… you are NOT crazy! And yes, it IS horribly heart breaking to hold your baby and not know what to do to help them. I too often cried with my babies.
My ped wanted my second son to start out with half of 15mg Prevacid a day and I was not impressed as I was sure that would not help. By the time he was 6 months old he was taking 30mg a day and it still was helping little. 🙁
You might want to consider trying probiotics and digestive enzymes for your little one. Here is my story on how a supplement of them helped my children.

the mucous in poo might just mean she needs elecare or neocate..

any rashes? cradle cap? baby acne? these can also be signs of protein intolerance/allergy…

She has stopped screaming but seems really uncomfortable. All the time. Still doing 7.5 Solutab once day. The doctor said “give it more time”

next: try TWO or even THREE doses of the prevacid. it might just happen ‘accidentally’ – you give a dose and, let’s say, baby’s father gives a dose, not realizing you did already- it could even be a ‘comedy of errors, due to sleep depravation that your mom or another person gives a THIRD dose, oops!!
if it WORKS, then you tell the dr your (crazy, sleep deprived family) story…
again: I canNOT tell you to do this, but you would NOT be the first
