InactiveI went to the marci-kids site but I am still confused. Can someone help me figure this out. I was told the boys are on the highest dose of zantac (at least they were before they started the prevacid). Here is how it went Dakota (17lbs 5oz) was on 1.5ml 3xday now is on 2ml 2xday (along with prevacid). Logan (13lbs 9oz) was on 1.2ml 3xday and now is on 1.5ml 2day (along with prevacid). Are these doses still right and what is the equation to figure it out so I can right it down to keep up with thier GI? Also the prevacid dose on the marci-kids site just confused the heck out of me. Here is thier dosage….Dakota- one 15mg solutab mixed with 10ml of water and split into two doses. 5ml at one dose and 8 hours later 5ml in another dose. Logan – one 15mg solutab mixed with 10ml of water and in one dose he gets 4mls and 8 hours later gets another 4ml’s. Is this right for their weights and age. They are 5 months old. Also what is the dosage amounts for them too so I can keep up with the doc for that too? As I said, marci-kids side, I just didn’t comprehend. Maybe it is twin mommy brain, I don’t know. Thanks for the help.

InactiveWell, my daughter is 17 pounds and on 30 mgs of prevacid split 3x a day. (are you spacing the zantac and prevacid by 4 hours?) Joel from marci-kids will be back next week, so you may want to repost. I couldn’t think with my sleep deprived brain either, the dosage just confused me!

Keymasterthis is what I know off the top of my head- this was what the doses ‘were’ with jonah- i think this is even more conservative than marci-kids, but it should give you an idea…
ok, prevacid is 1.5mgs per kilo at the ‘low’ side and it’s 3mgs (or even higher) per kilo on the ‘high’ side
so:let’s say Dakota is 17lbs (being conservative- he’s more!).
17/2.2 = 7.7 kilos
1.5 x 7.7 = 11.55 mgs per day, total, of prevacid on the low side &
3 x 7.7 = 23mgs per day, total of prevacid on the high side (can go higher)
so dakota’s current dose of 15mg for the day is in the middle and definitely has room to increase (& let’s not forget that he’s actually heavier than what I calculated for)

logan is 13 and 1/2lbs, so 13.5/2.2 = 6.1 kilos
1.5 x 6.1 = 9.2 mgs per day, total, of prevacid on the low side &
3 x 6.1 = 18.3mgs per day, total of prevacid on the high side (can go higher!)
so, logan currently is not getting a whole 15mg of prevacid & has room to increase
all that being said, I’m pretty sure my calculations are more conservative than the marci-kids site…anyone?

for zantac, it’s 5-10 mgs per kilo
dakota: 7.7 x 5 = 38.5mgs total for the day & the liquid is 15mg/ml, so 38.5/15 = 2.6mls total for the day on the ‘low’ side &
7.7 x 10 = 77mgs total for the day & the liquid is 15mg/ml, so
77/15 = 5.2mls total for the day on the ‘high’ side &
dakota get 4mls total for the day…so there’s room to increase the dose…also, it should be split 3x a day, I do believe…Zantac dosing:https://www.infantreflux.org/forum/forum_posts.asp?TID=1000
no time to do logan’s calculations – I’m sure there’s room for his to increase,too…

InactiveThe only problem with the zantac being split into 3 doses is that they eat only 4 times a day. Twice before they eat they get zantac and twice they get prevacid. I know they have to be 4 hours apart so there really isn’t more room for another dose of zantac. unless I give it to him in the middle of the night and I finally got him sleeping through the night again. If it is only in two doses, will that cause a problem?

InactiveLaura, doesn’t the chart at MARCI-kids indicate that these PPI doses should be given three times per day?? I think….
Dakotah shoud be getting 9.9 – 11.9 mg three times daily.
17.5 lbs divided by 2.2 makes him 7.95 kilos
7.95 kilos x’s 1.25 = 9.94 mg 3 x’s daily
or on the high side
7.95 kilos x’s 1.5 = 11.9 mg 3 x’s daily
Logan should be getting 7.7 – 9.2 mg three times daily.
13.5 lbs divided by 2.2 makes him 6.13 kilos
6.13 kilos x’s 1.25 mg = 7.67 mg 3 x’s daily
or on the high side
6.13 kilos x’s 1.5 mg = 9.19 mg 3 x’s daily

I don’t know how you will administer Prevacid and Zantac if each is to be given three times daily, but MARCI-kids does recommend that PPI’s be given to infants three times daily. Maybe if the boys were getting the right dose of PPI they could get by with less Zantac. Most moms here say PPI’s work much better than the Zantac.

There’s another PPI called Zegerid which doesn’t have to be given on an empty stomach. It may be easier for you to use given that your boys only eat four times per day and you are giving two medications. I’ve heard good things about Zegerid ( I wanted to switch my son to it but he won’t take it – he’s stubborn and I guess it doesn’t taste very good). Maybe it’s an option for you.

Keymaster(side note: my ped GI told me to space the zantac and PPI 2 hours apart…we never had a problem with this…however, as the PPI dose gets to be more ‘right’ you’ll find you don’t need the zantac…)
not sure about the marci-kids info- just pointing out that if you go by my ‘conservative’ calculations, there is room for a PPI increase- I’d guess that there’s even more room to increase from the marci-kids site…
