InactiveI know, Natalia, it doesn’t really make much sense.
Yeah, an overall greater amount of solution per dose would equal more mls of meds (especially when it’s a different concentration), but I don’t know how they can just recommend giving more b/c you need more buffer. Shouldn’t the recommended amount per day be based on how the med is metabolized?
My understanding is that when using the 20mg packets, you’re fine b/c you have enough buffer as long as each dose is 3.5ml. Does that mean that when using the 40mg packets (not with the mylanta) each dose needs to be 7ml or more to ensure enough buffer? If so then the dosing would need to be high per day for it to work.
The only thing that I can make of what he said is that they had to actually suggest higher dosing if using the 40mg packets (without the mylanta) in infants, because each dose would need to be at least 7ml to ensure enough buffer per dose (due to 7.4 percent sodium bicarb in 40mg packs vs twice as much in the 20mg packs). When recommending how much to give using the 20mg packets, they used the pharmacokinetics curve based on omeprazole, but it sounds like when recommending using the 40mg packets, they had to suggest higher values to make sure that each dose would be 7ml. That would up the total number of recommended mgs per day. (Gee, I think that’s as confusing as his reply! Not even sure if I can understand my own thought process! Anyhow, I’m just guessing that’s what he was trying to say).
Essentially, the way it was described to me, is that the 20mg packets (and the CaraCream JR kits) are like a stable solution made at 2mg/ml and 16.8 percent sodium bicarbonate. The 40mg packets (and the regular CaraCream kits) are like a stable solution made at 4mg/ml and I thought half the amount of sodium bicarbonate.
Anyhow, I guess I should just let it go, but it’s become almost like a riddle to me, LOL.

InactiveLori & Natalia:
Should I email your questions to him and see what his responses are? If you want me to, I would be happy to. I wouldn’t have to use your names if you don’t want me to. It would be nice to finally get a good understanding of how this works.
Just let me know.Robin

You use the 40mg packets? I don’t know then. I give up.

It’s okay Robin. I can email him… hopefully he doesn’t mind- I did just spend over 350 dollars on CaraCream!

InactiveI was a bit envious of you girls and your zeregrid- not having to time it around meals and all. But after this post I think I will smile next time I give my Zoton suspension to Alana! How confusing!!!!!

Inactiveactually im not confused lol.
its just that you have to give more mg with the 40 mg packs.anyway what is zoton? if its a suspension of prevacid then you dont have to time that with meals. any compound doesnt have to be timed.

InactiveZoton is prevacid-its basically the packets that are strawberry flavoured. To be honest, I don’t time at all. She gets them on an empty stomach and then within a few minutes gets fed. It is probably not as effective, but since being on it her reflux has virtually disappeared, so it must do something. My piglet wouldn’t wait 30 mins for anything- “I’m awake- feed me!!!” You do what you have to do.

Inactiveoh nevermind i thought you had the actual compounded suspension.
yeah technically, you are supposed to wait 30 min. with the packets before eating.
good luck.,November 23, 2006 at 11:35 am #18830Anonymous
InactiveOMG you guys!!! Now, you have me second guessing myself…… I was told that as long as I am giving 3.5mls (14mg) of Zegerid from the 40mg packets we did not need to buffer with the Mylanta. We are now up to 4mls (16mgs per dose.)
For awhile we tried the 20mg packets, but we still gave the same amt of mg’s per dose (more ml’s, due to the 2mg/ml concentration.) But, still the same amount of Medicine, or so I thought……
Did Dr. Phillips ever answer your emails? I am totally confused now, as I was always told by Joel that it didn’t matter what packets you used, you always gave the same amount of Mg’s per dose.
Is this not the case now?

Inactiveyou dont need mylanta jenay if you are giving a minimum of 3.5 ML.

InactiveI know, but I am questioning his doseage…

InactiveMy advice would be to email Dr. P with each of your situations and see what he says. It is all so crazy!!

InactiveJust wanted to let you all know that I have decided not to email Dr. Phillips, as I’m finding it too confusing, especially since we don’t use Zegerid.
Jenay, Joel explained it to me the same way, which is why I also don’t understand.
For those of you who are planning to send you questions to Dr. P, if you would post your replies, that might help others.
Good luck.
