Ive been lurking awhile and have gotten great info from you ladies…currently my daughter (3 months on friday) has reflux and is on prevacid 7.5mg 2 times a day, with some mylanta given during the night for relief…she is also on nutramigen formula….I guess I just have a few questions for you “vetrans” before I pester my dr. again.
My questions are
She has very runny stools, mostly liquid…and has a killer diaper rash that hasnt gone away with triple paste, antifungals, mylanta mixed with vaseline…
She has started eating less, and gets all squirmy during feeds again and now stops at 2.5-3 oz when she was up to 4-5 oz (shes around 13 pounds—she has had gaining and losing issues most of her short life)
She does have very sensative skin and the rash that she had forever that went away for awhile on nutra…is now back…
She sleeps in her carseat, and now even coughs, gags in there –but only at night or during her long nap.
Does this mean upping meds or different formula? What would you guys expect the doc to say (I am going to call him next also, but just wanted your input.)

What form of Prevacid? I would also ask to try Neocate…. The rash would make me think allergy…….

Inactivesounds like the prevacid dose is low. have you seen marci-kids.com? its a very informative website about PPI dosing you can also show your doctor. she sounds like she is in some pain.
also when you say the rash went away on nutri and now its back….there is a possibility that the milk proteins can build up in the system, and she may need an true hypoallergenic formula like neocate, which is what my son is on.

InactiveThanks for the fast replies..I just left a message for her dr also…
She is on solu-tabs with the prevacid,and takes them pretty well…I have actually self-upped them the last few days in hopes to help her sleep and be comfortable when awake, but it doesnt seem to be helping yet….
I will check into neocate…I wasnt sure if that would be right for her since Nutra worked (about 4 weeks on now) for awhile….ugh. –for me all I think of is gluten issues since my son has/had those..but I know that she is gf right now (only formula fed) this whole reflux thing is new to me!
Thanks again, and Im going to go check out marci-kids…I have seen that talked about on here…but I guess I figured she wasnt so severe as some, but I guess that may not be true….we had 2 really good weeks….

Inactive15 mgs of prevacid was enough for Dylan even in the worst of things. His issue was more formula. I think that I would personally want to try the new formula first because I would want my child on the lowest dose of meds possible. I think I would have felt like I was masking a problem with high doses of meds without finding the real issue. I think many babies need the Marci-kids recommendations, but I also think that many kids don’t. You have to do what you think is best and work with your doctor to find out the right solution.
Also, neither one of my children could tolerate the solutabs. Some babies do just fine on them, but my children were sensitive to something in them. Either the lactose or the aspertime, or the flavor…not sure, but either way, they made my children worse.
I think I would ask for Neocate first, and see if that’s the issue, and then possibly a different form of the med. I think we had the most success when we did one change at a time so we knew what was working.

My dd is just a couple weeks younger than yours. We are on 15 mg of the Prevacid Solutabs and we were trying to follow the rules I’ve seen on here and the marci-kids site about splitting it into 2 doses (because they metabolize it faster), giving it at least 2 hours after the last feeding, and at least 30 minutes before the next… But she was really roller-coastering. It really stressed us out but it seemed as though she was really sensitive to the time of when we gave her the two doses of meds. Often times she would get the AM dose around 4 AM (she’d wake looking for the pacifier and we would give her the meds then to meet the “30 minute before” requirement and she would fall asleep until 6 or 7) but because of her being in daycare already she wouldn’t get her PM dose until 5 or 6 and by then she was over the edge and it would take ALL EVENING to bring her back down.
Just recently we made two changes and they both seem to be making a difference for her:
1- We started giving her the full dose of meds at 4-6 AM. Now we don’t have to stress over the timing or that eveing dose and if the effect of the meds starts to run out before the 24 hour period then it’s in the time where she’s sleeping and not trying to eat (she doesn’t get a bottle between 10 or 11 PM and 6-ish AM).
2- We started doing the cereal in ALL her bottles. THe doctor had suggested this but she didn’t seem to like it and we didn’t really know how to do it right. But the theory is that the weight of the cereal can actually solve the reflux problem where the meds just make it where it doesn’t hurt. Anyway, we’ve been using a blender to make a batch of formula (24 oz warm water, 12 scoops of formula, 12 – 16 tsp of cereal) and now it’s real easy for her… to the point that the #2 nipples we are using are almost too fast but she doesn’t seem to mind and with me having a 2.5 year old, her taking a bottle faster and smoother makes us all happy.

Thanks for the tips..the timing for us is alright…she eats pretty much the same times everyday so I can get the meds in her 30 min prior (thats the one time her being a not so good sleeper helps, shes usually awake) She also still gets up 2x a night to eat, so for her giving the full dose in the am could make the overnight feedings bad…as it is she gets her 2nd dose around 9pm and still needs mylanta at her 3am feeding…
The rice thing sounds interesting…we tried rice in the bottles, the standard 1tsp per ounce, but she just got gassy and irritated so we stopped (and she was still spitting up….when she spits up its 2-3 hours after a feed and its clear fluid)
When you make your rice cereal/formula blend do you really only use 2 scoops of formula and then the rest rice? that is interesting to me..and depending on what my dr says I may try that for her….(less milk and soy)
Thanks for all your ladies help…Im still waiting for the return call…Im very glad grandma wanted the kids today…its nice to have time to look up info and catch up on stuff…

InactiveOH YEAH!!! It’s 12 scoops of formula! I better go back and change that!

We started out with using 1 tsp of cereal for every 2 oz. of formula but then I saw a video that someone posted a link for here and it indicated 1 tsp for every 1.5 oz of formula. Since we are now doing it in the blender I think we need the higher ratio so the last few nights I’ve used 16 tsp of cereal.

InactiveWe use oatmeal for Carter as the rice did the same to him, gassy and constipated. Oatmeal is working well for us…. Only beechnut as it is soy free…

Inactiveyes i noticed when i switched to beechnut rice as opposed to gerber rice the gas and constipation went away.

InactiveSounds like you’ve gotten terrific advice already. I agree about trying Neocate and also Oatmeal instead of rice cereal.
My son definitley needed the marci-kids dose. Things turned around for him quickly once he was on the high dose they recommend.
Good luck. I hope you find the right combination of meds and formula to turn things around soon.

We cant do oatmeal since oatmeal is a problem for celiacs, and it runs in the family so we are avoiding gluten for her for now…I do wish I could use it though…
I went to the doctor and he switched her to zegerid on his own…I didnt even have to mention it (he has also heard of marci-kids when I mentioned it)…so we shall see how it goes…Im kinda excited since I have heard such great things! We are waiting on the formula switch to see if the meds help, she is also getting an upper GI done on Thursday since she seems to be aspirating some also, and he wants to check what damage has been done in the esophagus….fun fun fun (ugh)
If the meds dont improve and if her diaper rash doesnt improve with his diaper rash concoction then we are trying neocate….(clortrimazole and bacitracin mixed with balmex if anyone cares) He says that alot of kids on Nutra get bad diaper rashes that arent always intolerance related…
