I am new here but I have been dealing with what I thought was mild reflux for about 4 months now.
Little History….
Aaron was born at 31 weeks with something called choainal atresia (sp) basically his nose was blocked with bone…not so fun..He was little 3 lbs 7 oz so we had to wait with him intubated in the NICU for almsot 2 months for him to grow and get the surgery to fix it. Got that done but at that point he started throwing up through is nose…..then they said ow my he could have had reflux this whole time and because he was intubated and and his nose was blocked we never knew…..so we started zantac and reglan and it seemed to help. Fast forward he would not extubate he had a very damaged airway which they now think was from the untreated reflux…so we got trached and started working with an ENT. Aaron came home on zantac and reglan and we got him eating and growning pretty good so we pulled the ng tube and went to all bottles. A month later he hates eating and vomits like a champ. So we work with the ped. to try some new stuff…. move the bed up and do some reflux precautionary things. A little improvement not much…..
Went to the ENT last week and aparently his airway is much worse and looks like severe reflux….so we had a GI doctor at CHOP (childrens in philly) check him out and they did upper GI to see if he had all his parts in order..that was normal and he also had a swallow study done and that was normal while in the NICU.
So they trippled his prilosec took him off zantac and then said we would come back in a few weeks.
I feel so bad he hates to eat and he is gaining weight so slowly. We give him alamentum with cereal in it to max his calories and try to keep things down. He will only eat in his sleep now….so weird and never takes large amounts.
Questions:
What else could I be doing?
Any formula suggestions?
Who are the really good GI docs? (in the greater Philly/Hershey PA area)I just feel like if I could get his reflux under control and get him to eat and gain weight we might stand a shot at getting rid of this god awful trach.
Any suggestions would be great!

Sounds like you’ve been through a lot with your little boy. It’s too bad they didn’t find out about his reflux earlier.
His symptoms, only sleep feeding and taking only small amounts, are so classic of reflux. Hopefully that will improve once the Prilosec starts working.How much Prilosec is he taking? How often?
How long has he been on the higher dose of Prilosec? Most say it takes two weeks to begin working, but maybe a shorter time when a dose is uppped instead of just started.
Some doctors recommend continuing Zantac until the PPI begins to work; giving the two drugs 2 – 4 hours apart from each other.
Also many moms use Mylanta Cherry Supreme before feeds or sleep.
I hope he is feeling better soon, and I’d like to suggest that as soon as you see some improvement, and if he is willing, try to feed him awake and avoid sleep feeds as this can lead to some very difficult challanges later on if he becomes accustomed to only feeding while sleepy and avoids eating at other times. If he continues to only sleep feed, it’s likely that his medication is not working, so you should probably talk to his doctor about it.
I hope Andrew is feeling better soon. He’s sure been through enough.

He gets 2.75 mls of prilosec 2xs a day. It seems to be helping a bit. He does not vomit quite so much but still no appitite.
I will try the mylanta…..how much?

welcome. it seems like your little one has gone through so much. I am sorry about the surgery and the trach. He is so young and has gone through so much and now… the evil reflux.
It sounds like reflux. First of all, don’t let doctors fool you and tell you because he had a clear Upper GI then there is nothing wrong with him, or even a swallow study. The upper GI will show whether milk is going the right way and wether there are any hernias (sometimes only) and whether food is clearing the stomach (only for a few minutes, which in itself is not relevant as some babies have slow motility and the slower emptying happens later in the meals). If they want to prove reflux they have to do a ph probe and an endo possibly. The ph probe is like an NG tube with a sensor at the end that will show the duration and incidence of reflux episodes. The endo (a tube down the esophagus and into begining of duodenum) can tell you whether there is erosion of esophagus, ulcers, EOS cells (inflammed white blood cells from food reactions for examples), any type of thursh, bacteria etc. If Aaron is not improving soo and not gaining weight, please demand these asap, before it is too late.
Matthew was a lot like Aaron. Nobody paid much attention for a few months, nobody explained to us what tests could be done etc. For maybe 2 months we sleep fed him; also he would stop at a magical 2 ounces all of the time, limiting his intake. He never really threw up until later and we still have no idea if it is behavioral or not. We did nothing until it was too late. When we tried to stop sleep feeding, he lost weight and we had to have an NG tube. He now eats very little by mouth and maybe 75% by tube? Depending on the week and we have an appointment for a gtube on the 26th.
What would i do differently?
1. research the amount of prilosec you can give. There is a website called marci-kids.com. check out the dosage there and fight with your doctors. the amount you are giving seems little (what is the concentration?)
2. start asking about the tests we discussed. See if they are willing to do them. If he is throwing up, ask for an emptying scan. This will tell you if his body is digesting food too slow
3. try to feed while awake. even small quantities. Make sure he has a good time and yuo do not force it into him. If he feels better and starts liking to feed, he may turn around on his own. Try, try please. It is important and i wish i had known.
4. what is he taking? have you considered that his reflux may be aggravated by the formula? Discuss with your ped please. Oh i see he is taking alimentum. Does he have any reactions that you can see? Is it worth a trial of neocate?? Please discuss with teh GI
5. if you cannot feed him awake, and he is not gaining… well you know about the NG tube… we wish we had it earlier. We waited until we had a full aversion adn it is going to take at least a year to get matthew off his tube now. If you can get him to gain weight, then it is fine and some people have done well with sleep feeding etc but given my own experience, the tube is a good way to get over the bump
I am sorry for the long post. I feel terrible for you and i am praying that the trach comes out soon. PUSH your doctors, go with a list of things, ask why and why not, how…

Actually the PPI drugs don’t really help with vomiting, but they SHOULD help the pain. They are supposed to stop the acid production so it doesn’t back up into the esophagus and burn. My son’s vomiting comes and goes- some days are terrible, and other days he doesn’t vomit at all. But on Prevacid his pain is controlled so he sleeps well and eats pretty well, though he does have some issues (he prefers to be spoon fed to bottle fed).
Here’s a link where you can check on PPI dosing. Many doctors do not prescribe these drugs in high enough doses for infants because they are not aware that infants metabolize these drugs 3x’s faster than adults and that they actually need higher doses given 3 x’s per day. http://www.marci-kids.com/
I forgot to ask how much your son weighs, but you can figure his dose by converting pounds to KG and checking the chart for OMEPRAZOLE (which is the drug name for Prilosec).
I’m sorry I don’t know the dose for Mylanta but you can probably find out by doing a search on Mylanta or posting a question about it.

InactiveMylanta – again, how much does he weight? When Matthew was 11 pounds or so, they said 2.5ml up to 6x a day… so a LOT.

He gets 2mg/per ml at 2.75 two times a day we go with 6ish for both b/c his reflux really peaks at 7 PM, like clock work he is almost 12 lbs and came home in feb at 8 lbs so he is gaining just so very slowly
I did not understand the dosage stuff but I am going to work it out tonight but I did read that it should not be flavored and our local target is who compounds it for us flavors it grape……
oddly tonight for the first time in a week he has had an awful reflux day…especially tonight poor guy i wonder if its b/c the stuff is a week old and now is not working properly I am calling in the morning.
We are scheduled to follow up with the CHOP (children’s in philly) in a month for a ph probe and endo…they met with us while he was an inpatient last week for airway stuff.
