Hi all,
So this is my 4th reflux baby (second with Zegerid) and I should probably know this by now, but I’m a little befuddled so I’m coming to you all for help.
My last baby was on Zegerid, the MARCI kids dose. I thought I’d read that you add 6.5mL water to a packet, pull your dose, and add equal parts MCS. So, I was giving 3.5 mL of the Zegerid that had been mixed with the water plus 3.5 mL MCS 3 times a day to my daughter (she was a year old at the time; I don’t remember the exact weight).
My newest baby is now almost 3 weeks old and 10 lbs. I’ve been refreshing my mind and see that I was mixing incorrectly, at least I think I’ve been doing it wrong.
For my newest LO, I should be giving her 4 mL of Zegerid 3 times a day. However, I’m seeing that I should be mixing the packet with 6.5 mL water, adding 10 mL MCS, and then giving the 4 mL, right? So not 4 mL of just Zegerid plus 4 mL MCS (for a total volume of 8 mL), but 4 mL of the mixture of Zegerid and MCS total, right?
I was giving her 3.5mL of just Zegerid and adding 3.5 mL of MCS to the sryinge and giving that 3x a day. She was gassy at night, but otherwise, eating and sleeping well. I realized I was doing it wrong, made the new mixture of the combined Zegerid with MCS and started giving the 4 mL total of the combo yesterday.
Since then, she’s refused almost every feed and stopped sleeping as well. She’s a silent refluxer, but when she refluxes, she’s really upset now where as 24 hours ago, she was tolerating the episodes much better. She’s way more fussy and writhing almost continuously now too.
I was hoping by correcting the dosing, I’d reduce the gas from the super high dose of Zegerid she was getting, but still manage the reflux.
Should I just give her the 4mL of pure Zegerid and add the 4mL of MCS since that was working? Is this withdrawal and will correct itself in a few days? She doesn’t have loose stools – oh, she’s on Neocate already so I know we’ve got the food allergy problem eliminated – and her poops are normal so I’m not concerned about the high dose of MCS.
Feedback would be great; even though I’ve ridden the roller coaster with each kid, I could still use some info!

There’s mixing for the 20mg packets and also the 40 mg packets- could this be the confusion?
or are you using zegerid OTC?

I figured I should go back to what was working. The change was really drastic!

March 15, 2013 at 12:24 am #71832AnonymousInactiveI was reviewing the OTC Zegerid mixing thread and saw this blurb on Zegerid 40 mg packets:
“Joel in one of his previous posts (that I believe is stickied) said, “The 40mg packets of Zegerid contain proportionally half the amount of buffer as the 20 mg packets, so when using the 40 mg packets, mixed at a concentration of 2 mg/ml, a minimum volume of 7ml (14 mg) should be administered per dose.”
This clears it up a bit for me. Thought I’d pass it along in case someone else runs into the same issue when mixing the prescribed 40mg packets.
And, the question was also asked:
So would this 7ml volume be required for the 20mg OTC Zegerid capsule plus 20mg omeprazole capsule cocktail?
I’m going to assume this to be true since I found giving a total volume of 3.5 mL (of Zegerid and MCS) to be too little for my girl. I’m going to give a total volume of 7 mL of the OTC mixture, too, just to prevent the same problem from occurring.
