angry with GI appt today

We saw Sophie’s GI today. We have seen him since she was born, 23 months ago. For the most part, I really like the guy, but was kinda upset today.

She is still not sleeping much at night and acting like she has tummy aches. She has more loose stools than I KNOW is normal. She was scoped 8wks ago, no EE or EOS. Just bad reflux.

He said she is on a good dose of Zegerid and wont go higher (30mg) and he said to do zantac before bed.. which we are. He said 2mls (that seems kinda low) and he said it can be given ANYTIME after the PPI. I heard 4 hours, he said it doesnt matter. Is he wrong?

He saw her butt rash, said he doesnt know what it is. Could be from food, could be infection. HUH? He couldnt help. Said to see her DR for cultures.

Story with rash. Had some elecare, broke out in rash. Went to DR, said it was staph (just on her butt) and was on augmentin. Cleared up. 2wks later, tried some elecare, same rash, back on meds. Clear for 1 week, now its back (no elecare trial) I thought it might be corn syrup, she does get weird poop if she eats candy, which isnt too often.

She is very intolerant of dairy, could only drink neocate as a baby. We do avoid dairy.

Dr Phillips created zegerid, and he said you have to give an H2 blocher 4 hours apart from zegerid.

is she on 20 mg packets or 40 mg packets? (the dosing is different for each packet)
have you checked her dose at amarci-kids.com?
have you shown the doc the information from marci-kids.com?

I didnt show him or mention the marci-kids website. She is on the 20mg packets and I make them with 6.5 ml’s of water. She gets 5 mls’s three times a day. Why was he so quick to say it was okay to give zantac with it?

So, if im doing it three times a day, and she goes to sleep at 8pm, how do I get the zantac into her? Should I just do two doses of 15ml’s??

About the rash, Liza had a rash once that looked a little like that. I tired everything and finally her doctor said to use Triple Antibiotic ointment and it cleared right up. I never would have thought to try that but it worked wonders.

Are you using the same diapers all the time? Sometimes babies can react to certain diapers so if you buy different brands that could be a problem. My babies used to break out when I used pampers.

Definitely wait four hours between Zegerid and Zantac.

The working dose for Zantac is 5-10 mg per kilogram.

Is she MSPI? That could explain the loose stools as well as the tummy aches and poor sleep.

Sorry that your appointment didn’t go better. I hope the Zantac before bed helps her to sleep better.

Well, she is MPI, maybe soy, I dont know. I guess ive never cut out soy. She doesnt eat soy milk, cheese, yogurt, etc… but she does eat hidden soy, a lot.

Ive thought about cutting out soy, but she wouldnt eat anything then. She only eats about 4 foods, all contain soy. 🙁

We havent switched diapers. We use 3 kinds, and have since she was born without issues.

Sorry to hear your appointment didn’t go well. I hope the zantac at night does the trick for her. Maybe if it doesn’t work after a few nights you could call and ask to try a higher dose? Good luck with the cultures tomorrow. Hopefully that will give you some answers. Those staph infections can be hard to get rid of (if that’s what it is), and they kinda come and go like that. It does sound like a food intolerance from what you’ve described though. Hopefully you’ll find out tomorrow.

Zantac dosage is 5-10 mg per kilogram she is 10.45 kilograms. The high dosage would be 104.5 mgs per day and on the low side would be 52.25. Most Zantac is mixed 15 mg per ml. The high dose would be almost 7mls. and the low would be about 3.5mls. Is it possible she has a yeast infection on her butt? I haven’t seen a rash like that but I can tell she would probably be very uncomfortable. Sorry.

They said it wasnt yeast, never responded to yeast treatment and she isnt bothered by her bottom AT ALL!

Thanks for the info on the zantac. Ive been giving her 4 ml’s! 🙂

Yes, that could be a possibilty as well (a yeast type diaper rash). All three of my babies have had them. A little monistat twice per day will likely get rid of it if that’s what it is. My doctor always says to continue with the monistat for a few days to prevent it from coming back.

Here’s a recent thread with some soy free food ideas. Perhaps eliminating soy from her diet will help. I was surprised when I first starting reading here that many babies are very intolerant to soy and they have quite severe symptoms from even small amounts of soy.

my daughter has had rashes like that before. when I took her too the ped they said it was a yeast infection, and they prescriped nystatin…have you tried that? It can take up to two weeks for it to go away with the cream…so you have to use it for a while.

Two DR’s have said it WASNT yeast. I dont think its yeast either. Okay, so if it is, why does she ALWAYS get it then?? Im so frustrated.

Its open like sores, they are quite large too.

This is the same weight as my son and he has the same dose of 2 mls of Zantac so it seems ok to me.

I really hope that you can figure out what will help her. The GI drs can be very frustrating sometimes!
