InactiveI am hoping some of you can draw upon your experience for me…
Here’s the situation at the present moment
Baby 6 weeks old.
Was spitting up 2-4 times after every nursing gaining slowly (4.5 oz/week
on breastmilk only, 6 oz in three weeks on 1/2 formula 1/2 breastmilk)
Started on Reglan and Zantac with no change.
Switched to Reglan and Prevacid at the same time switched to breastmilk
thickened with rice cereal.
Began to gain weight but I’m not sure which was more important- the
medication switch or the thickened feeds.
Rice cereal not working anymore because of amylase in breastmilk so
formula feeding Alimentum with rice cereal until SimplyThick arrives
tomorrow.
Pumping some to keep milk-in. Really hate pumping… what is the least
amount I can do to preserve it for the future when baby may be able to go
back to breast? Miss nursing.
Have to use more rice cereal than doctors’ instructions in order for baby
to keep anything down. What are the drawbacks to rice cereal?
Will I notice an improvement with expressed breastmilk and Simplythick
over Alimentum and rice cereal?
On dairy free-diet one week and not really reading every label. Don’t have
much time to research allergy as pumping, bottlefeeding, holding after
feeding and cleaning take so much time by the time its done its time to
do it all over again. Family history of milk allergy. Do allergy tests help?
Which ones? Is there a diet I should start with and then add foods?
When reflux is this bad will the baby ever be able to nurse directly from
breast again? Does the milk have to be dumped until diet has time to kick
in? When will that be? Will baby develop allergy to Alimentum after
awhile? Vomiting first appeared after switching to half breastmilk half
formula due to slow gaining only to gain more slowly until rice cereal
added.
Did any of you quit breastfeeding only to regret it later?
Is it important to find out why the baby is spitting up in the first place or
is it only important that the baby gain weight with thickened feeds and
forget wondering about why he needs thickened feeds to begin with?

InactiveHi there, there are quite a few people on this site who have been able to successfully BF their gerdling. Hopefully they can provide you with some hope. I also had problems BF my daughter b/c of her feeding aversion. I pumped every 4 hours, and my supply still dwindled. It was just so hard to keep it up by pumping alone. I did find that adding herbs (blessed thistle and fenugreek) helped to boost my supply. I even considered adding domperidone, but found that in the end, I just didn’t have the time to pump enough. I was sad when I had to stop BF, but I knew in the end that I had no choice, and I never looked back. I always think that it was actually Hailey that made the decision to stop in the first place. I hope that others on this site can offer you more information to help you. I do know that people have been able to continue BF their refluxers. Good luck to you.

At lalecheleague.org it references a study that showed moms who nursed
under 8 times a day had high rates of lactation failure…and with a pump not
being as good as a baby…sigh…if it was posssible for the baby to gain
weight appropriately with only some of the feeds thickened that would
help..has anyone tried a combination of thickened feeds with regular
nursings at the breast with success?…it would be nice if this were possible

InactiveI don’t know if this will help you at all but I also noticed that adding cereal to breast milk doesn’t seem to work. I didn’t realize it was because of amylase in breast milk. I currently make bottles for my son using 4 ounces of breast milk, 3 ounces of formula (soy), then add 4 – 5 tablespoons of oatmeal (not rice) cereal. After lots of experimentation I found this to work. (He doesn’t drink the whole thing). I do this so he gets more breast milk than formula. My son would not nurse so I pumped for 4 1/2 months and froze lots of milk for him. Pumping is very difficult, so I know what you are going through. I’ve done it for three babies (none would nurse for a variety of reasons). I hate to say it but from my experience you have to pump a minimum of 8 times per day to keep a good supply at least early on, later you can reduce to maybe 6 or 7 times.
I know it’s horrible, but you’ll never regret doing it. You have to decide when you’ve done all you can do and quit pumping. No one can make that decision for you. With each of my children it was different. I did it for 5 months for my first, 6 months for my second, and 4 1/2 for my third. If you’re baby becomes constipated you may want to ask your doctor if you can switch to oatmeal. I started all my babies on oatmeal. My doctor said it was fine.*** My doctor also suggested a dairy free diet for me when my son’s problem first began, but I wasn’t perfect with it either. Where do you find the time between pumping, feeding, cleaning up, etc. etc? For me it turned out that once my baby was on the right medicine (Prevacid) his symptoms improved so dairy was probably never the problem. It’s complicated when you have so many factors to consider. Is it important to know what’s causing the problem? Probably yes, but sometimes it’s not possible. Some babies probably have a variety of problems that contribute to feeding/spitting problem. Does your doctor think the baby has REFLUX? Was the baby refusing to eat? Arching the back during feedings? Throwing up large amts of food? Fussy after eating and having trouble sleeping?This is what I would do if I was you. Check out this website http://www.marci-kids.com/ to see if baby’s symptoms sound like REFLUX/GERD. Also, check out the PPI dosing information at this site to make sure your baby is getting the right dose. (Many doctors prescribe too low of a dose for infants). With the right dose it could take two weeks for results – my son improved in less time, 1- 1 1/2 weeks once he was on the correct dose. In the mean time maybe try to stick to a dairy free diet as best you can, pump and freeze, but feed only formula. (It takes time to clear the body/breast milk of dairy- I know, more frustration
). Then if baby improves on medicine, try breast milk again. If baby does well then try adding dairy very slowly to your diet again to see if baby can handle it. The problem with this is that after some time your baby may not return to nursing after getting accustomed to bottle feeding. Maybe do the opposite and go back to nursing first (being strict with your dairy free diet) to see if the meds are working, then if the problem continues, try formula again. You said the vomiting didn’t start until you started formula? So maybe breast milk was not the problem.I know it’s very complicated. I’ve been there. It’s frustrating beyond description, but we mom’s will do anything for our little ones. I hope I’m not discouraging you any more. I’ve been there, and I know the desire to nurse, the inability to do so, the frustration of REFLUX, the hardship of pumping. I also know it all passes and you will get through this, just as I am getting through it with the help of all of these wonderful moms on this forum. If you have to and choose to continue pumping I’d be happy go give you advice and encouragement. Inactive4.5 oz a week doesn’t sound like poor weight gain. what does your baby weigh? personally, i would not give up breastfeeding without doing an elimination diet and making sure that my baby was on good meds. breastmilk is best for reflux babies because it is easier to digest. sounds like your baby was gaining better on breastmilk alone. does your baby seem to be in pain?

I have a lot to say on this subject- these doctors who tell you things like to thicken your breastmilk get me SO MAD

I would get my baby on a ‘good’ dose of medicine and then I’d try the elimination diet (actually I DID do this!)
you might not even have to thicken- I realize it might seem necessary bcse of lack of weight gain- please get to a lactation consultant- maybe you have OAL? Over Active Letdown, also known as foremilk/hindmilk imbalance- maybe baby isn’t getting enough fatty hindmilk??

I will follow through on all the links as soon as I can…
For now I’ll say I think the problem may have started when a ped.
recommended 24 hours of bottlefeeding for what was probably just plain
old newborn jaudice…
The baby was not back to his birth weight by 2 weeks but he had gained
from his lowest weight…
I was mistaken when I said 4.5 oz in a week on exclusive bf…it was more
like 4.5 oz in 10 days…
then he was put on 8-10 oz of formula per day plus bf which is when the
vomiting started…
he vomited at least 2 times after every feeding sometimes as many as 4
times and just returned to birth weight at 5 weeks of age.
Then we did test weighings with a Babyweigh scale after breastfeeding to
see if he could return to exclusive breastfeeding but even with formula
out of his diet he couldn’t stop vomiting so around six weeks of age he
was hospitalized.
He was put on zantac and reglan and then after two days with no
improvement he had to have breastmilk thickened with rice cereal plus
the zantac was switched to prevacid which worked in that he has gained
14 oz in 1 1/2 weeks…
But he is still in pain and still vomiting…and i don’t know if the thickened
feeds helped or the prevacid or both…
The Simplythick should arrive tomorrow…any advice on how it can be
used? Has anyone tried thickening just an ounce of EBM and then nursing
directly from the breast?
Has anyone withdrawn thickened feeds after they seemed to help and
discovered it really was the meds that were helping not the thickened
feeds?
Can anyone give me a list of easy to prepare or just grab and eat foods
that are low allergy foods while I’m learning about this allergy stuff?

Baby weighed 7 lbs 8 oz at birth
7 lbs 4 oz at 2 weeks started on 8-10 oz Lipil w/iron spitting up
escalates to vomiting
7 lbs 3 oz at 2 1/2 weeks switced to 8-10 oz Gentle Ease
7 lbs 10 oz at 5 1/2 weeks referred to lactation consult
Lactation consultant calculates baby’s lowest weight to have been
probably around 7 lb 15 1/2 oz based on 7% normal weight loss so that
would mean the baby gained, conservatively, 4.5 oz from day 4 until the
2 week check up. LC’s opinion is that 24 hours of formula feeding for
jaundice is not proper, baby should have had photo therapy instead.
Thinks baby could have had silent reflux in the days following the 24
hours of formula because weight gain on the slow side for bf baby.
Consult and 24 hours of test weighing revealed milk supply to be much
better than you would expect with so much supplementation…LC opinion
that milk supply reflects what baby was taking, and baby was taking so
much from both breast and bottle to try to compenate for all that is lost
through so much vomiting.
LC referred back to ped for reflux, but doc doesn’t prescribe them so LC
calls doc with concern about feeding aversion developing if nothing done
b/c baby in pain. Doc sends us to hospital for pyloric stenosis…LC
doesn’t think pyloric stenosis b/c no projectile vomitng …ultrasound
doesn’t show p.s. then barium swallow shows reflux then meds started-
no change- so switched to prevacid and reglan and thickened feeds all at
once…vomiting reduced weight gain picks up…now at seven weeks old
baby 8 lbs 8 oz…
Can I find dosaging information at any of the websites listed, maybe I
should start out trying to find out if baby is on best dose…don’t want to
have to thickened feeds would like to return to bf at breast…

KeymasterSince you mention you don’t want to do thickened feeds and you want to return to breast:
This is what I would do (And I’m just a mama who’s hung around this board for almost 3yrs now, also the yahoobreastfeedingreflux board, and I’m not a medical professional of any kind
):I would get back to your LC bcse she sounds like she knows what she’s talking about
. If she has any moms groups, that might be nice, too, to find some support through all of this…I would stop the reglan until you know for sure your baby has Delayed Gastric Emptying. This is what reglan is for, and there is a TEST for this, for DGE. If your baby actually has DGE, which is possible, then we can help you with that. First things first
.I would check your baby’s dose for zantac (will do that for you- see below) and, if you have any left around the house, I would give this. Just make sure it’s given at least 2 hrs seperate from any PPI (Prilosec or prevacid- I forget what you said). Some doctors say 4 or 5hrs seperate. My ped GI said two hours & we never had any problems. Why give zantac, too? Because my guess is that the PPI dose isn’t ‘proper’ and I’d also guess that perhaps no one told you how to administer it correctly…So, in the meantime, it couldn’t hurt to get some zantac into your baby.
As for stopping thickening, plenty of people have done this.
The meds will NOT stop the spit up, but it will stop the pain. In theory, if the spit up isn’t painful, then baby will perhaps just want to eat soon after spitting up, thus making up for the extra calories. If this doesn’t occur, well, again, we’re here to help you and so is your LC.
As for milk/soy protein allergy/intolerance: are baby’s stools ‘regular’ breastfed baby stools, ie: mustardy and seedy? I’m guessing not. I’d guess that they are watery, green, mucousy (stringy) or foamy like shaving cream…These are signs of either a milk/soy protein allergy/intolerance or OAL.
As for what to grab to eat: pear, pear juice, yellow squash, free range ground turkey and lamb…that’s the core of the elimination diet…From the mspi link from my welcome/intro there’s a whole section about the elimination diet & I’ve posted there all about my own experience/thoughts, etc.

Zantac:
baby is 8 and 1/2 lbs. That’s 3.9 kilos. Zantac dose is 5-10mg per kilo. So, at a ‘low’ dose, that’s 3.9 x 5 = 19.5mgs per day. The liquid is 15mg per ml, so that’s 1.3mls a day, total, on the ‘low side’ and
at a ‘high’ dose, that’s 3.9 x 10 = 39mgs per day. The liquid is 15mg per ml, so that’s 2.6mls a day, total, on the ‘high side’

Ist I’m going to check out the MSPI from your welcome intro
2nd- can’t give the baby any Zantac because that was administered in the
hospital and was changed to Prevacid before we left so only have
Prevacid…
3rd…want to stop Reglan b/c of possible side effects but I’m afraid to just
yet…afraid to give it- afraid to take it away- what a quandry…
For now I don’t think I should make a change because if I make this
change at the same time I’m switching from formula+rice to BM
+Simplythick I won’t be able to separate out what is what..
if things get better or worse I won’t to know what is responsible for the
change…so I think I’ll try BM+simplythick first and then if that goes well
I’ll need help tackling the Reglan issue…and thank you for being willing to
say give it a try! It’s hard to get that kind of candor…
From reading the Marci site I gather there are at least 3 problems with
Prevacid…its no good after a week in the frig…it doesn’t have enough
buffer to work well in the tummy…and it probably isn’t the right dosage…
So I guess I should just lobby for a trial on Zeregrid when I’m ready to
stop Reglan…

coughing -gaging sounds…I see STRIDOR as side effect of Reglan…is that
what he has!!!
Going to stop Reglan and call doc…
Prevacid dose is currently 1 ml 2X a day (3mg/ml)
From MARCI site looks like dosing should be from 1.5-1.75 mg/kg 3x a
day
doing the math on line…8.5lb /2.2=3.86kg
so 3.86kg x 1.5 mg/kg =5.79mg-low dose
or 3.86kg x1.75 mg/kg =6.75mg-high dose
so that means the dosage should be about 6 mg 3x a day instead of 3mg
2x a day…
Have I got that right?
Will the doctor know this?
If the doc already knows this is he underprescribing the Prevacid because
the baby on Reglan????
Just an aside- should I be moving off this HELP!!! thread now and posting
particular questions to more appropriate threads…I’m new to chat room
etiquette so I don’t know if I’m doing this correctly…

InactiveThe only way I could get my son’s doctor to prescribe the right dose was to print the information from MARCI and send it to her, then talk to her on the phone. It seems most doctors don’t know the current research regarding PPI dosing for infants. It’s maddening…having to find the info yourself and convince your doc, but it’s all worth it. My son improved so much once he was on the right dose for his weight. (Your math looks good to me).
I don’t know anything about Reglan, and I’m new here too, so I’m not sure about moving off the HELP thread either.
I hope you get your little guy on the correct dose soon and that he improves quickly. Once my son was on the correct dose he was better in a week to a week and a half. Most people say it takes two weeks. Maybe it was faster for my son because he was at least getting some Prevacid for several weeks already.
